This form is to be used if a player is moving from one OMHA Association to another OMHA Association prior to the start of the new
hockey season, and as a result of the move will no longer be eligible to participate where he played hockey the previous season (SEE
OMHA POLICIES AND PROCEDURES 3.8, ATTACHED). If the pending move is from an Association that is member of an OHF
Member Partner other than the OMHA (GTHL, Alliance, NOHA, etc.) permission to tryout must be obtained from the Executive Director of
the OHF and completion of this form will not be required.

minor programs in the OMHA, OHF and Hockey Canada and are aware that these are available upon request. We also recognize that the falsification of
any registration document will result in the suspension of a minimum of one (1) year to a maximum of three (3) years for the above player and any team
officials involved (as per OMHA Regulations 8.3 a) and 8.3 b), OHF Regulations E8) and E9) and Hockey Canada Regulations F22 and F23.

PRIVACY STATEMENT: The information requested on this form is required by the Ontario Minor Hockey Association (OMHA) and the Ontario Hockey Federation (OHF) (their
respective executives, employees, coaches, trainers, referees and volunteers) for registration purposes and to administer the rules and regulations of the OMHA and provide notification of
any upcoming events or other activities. In order to do so, the OMHA, its Member Associations, OHF and Hockey Canada may, if required request proof of a player’s identity, address and
date of birth.

          3.8 Permission to Attend Tryout When Moving Prior to New Season

If a player will be moving prior to the start of the new hockey season, and as a result of the
move will no longer be eligible to participate where he played hockey the previous season,
the OMHA Vice-President for the District of the intended move, on the recommendation of
the OMHA Regional Executive Members for the Areas from and to which the player resides
and will be moving, may grant temporary permission for the player to attend tryouts in the
new residential Centre and/or its AAA Zone, provided that all information regarding the
proposed move has been submitted to the OMHA Vice-President and involved Regional
Executive Members by way of a TEMPORARY NOTICE OF INTENT TO MOVE Form.

This temporary permission is not a determination of change of residency nor
acknowledgement of a changed residency. Applicants are required to provide an OMHA
PLAYER TRANSFER FORM FOR RESIDENTIAL MOVES and all finalized supporting
documentation, as required with that Form, to confirm that a residential move actually has
taken place. These documents must be provided to the OMHA approving Regional
Executive Member by the date of occupancy of the new address or by September 1 of the
new hockey season, whichever is earlier. Once these documents are received, and the
Appropriate Regional Executive Member has approved the transfer, the process will be
completed.

If the applicant is not able to provide confirmation of residential move by the noted date, the
applicant will be required to return to his previous residential Centre and/or AAA Zone.

NOTE: A player is not eligible to obtain an AAA Waiver while attending AAA Tryouts with
Temporary Permission, nor is such a player eligible to participate in any exhibition or
tournament games while under temporary permission status.
