       2. Patient Care and Related Activities

2.1.     Out-patients Section
As in the previous years, a very large number of patients attended the OPD of the Institute
during the year under report. The trends in OPD attendance during the last 5 years are
given below :

The total number of new patients of who attended the OPD in the year 2003 was 8557 of
which 4557 were cases of leprosy and the remaining were patients having other disorders.

2.2.     In-patients Section
During the year 2003 , 794 patients were admitted to the In-patients ward for management
of reactions, steroid dependence, ulcer care and surgical correction of deformities.

2.3.     Surgical Care
The surgical section provides routine ulcer and wound care, corrective surgery and
physiotherapy services to the out-patients and in-patients of the Institute. 4918 patients
were seen in surgical OPD during 2003 and of these 261 cases were hospitalised for various
surgical problems.

2.3.1. Surgical Procedures and Related Activities
Corrective surgical services include reconstructive surgery for correction of hand and foot
deformities, peripheral nerve trunk surgery for pain relief and to facilitate sensory motor
recovery in hands and feet, wound and ulcer related procedures and other minor surgical
procedures. Besides the above, ulcers and wounds of outpatients are dressed and minor
surgical procedures are performed in the dressing room.

2    Besides this 11160 ulcer dressings including ulcer related minor surgical procedures were
     performed on out-patients and in-patients.
     Many patients were prescribed protective foot wears made of microcellular rubber with
     suitable modifications and several cases were prescribed fixed ankle braces with PTB from
     different sources. Needy patients were provided with static and dynamic finger and thumb
     splints. Modified drop foot orthoses were also provided when ever required.

     2.3.2.     Physiotherapy Section
     Physiotherapy section provides services to the patients referred from out-patients clinic as
     well as in-patients ward. They come to physiotherapy section for
     (i)     Evaluation of neuromuscular and kinesiological functions of the hands and feet
             during medical and surgical treatment.
     (ii)    For splinting and supervised exercises before and after corrective surgery.
     (iii)   For therapeutic purposes cases of acute neuritis, arthritis and arthralgia come for
             appropriate splinting and heat therapy; cases having plantar ulcers for POP cast
             application and patients having large joint contractures for traction and exercises.

2.4.     Radiography Unit
526 radiographs were obtained for diagnostic purposes. Of these 141 were chest
radiographs, 209 upper and lower extremities, 135 spine, 23 abdomen including KUB
region, 10 skull and 8 others. In addition 12 fluoroscopy were also car ried out.

2.5.     Histopathology
92 skin and nerve biopsies from leprosy patients were taken for histopathology during
2003. In addition, several research specimens collected earlier were processed.

     2.6.     Biochemical Investigations
     A total of 4800 investigations were carried out in the Biochemistry during 2003. These
     include blood sugar, blood urea, total serum proteins, A/G ratio, thymol turbidity, SGOT,
     SGPT, serum cholesterol, bilirubin, DDS in blood/urine, serum creatinine etc.

     2.7.     Clinical Pathology
     7929 specimens were examined during the year. Of these 6128 were skin smears for AFB,
     804 hemograms, 719 urine samples and 239 stool samples. 39 sputum samples were also

2    examined.

     2.8.     Microbiology
     Microbiology laboratory provided various types of diagnostic support which included
     culture and sensitivity (for pyogenic organisms and mycobacteria), serology for infections
     like typhoid, in vitro viability by ATP bioluminescence, PCR diagnostics etc.

     2.9.     Immunology
     Immunology laboratory provided facilities for serology for hepatitis, blood grouping and
     serology of leprosy.

     2.10.    Experimental Leprosy
     Service support provided by this laboratory includes viability and sensitivity screening for
     leprosy by mouse foot pad.

     2.11.    Central Sterilization Laboratory
     This laboratory continued to provide facilities for cleaning and sterilization of glasswares
     and sterile vials with and without anticoagulants for the collection of blood samples from
     the patients.

     2.12.    Social Services
     As in the past, the Medico-Social Unit continues to look into the social problems of the
     patients and makes efforts to solve them. Difficult patients are given necessary advice
     about the disease and their family members are given information about the disease and its
     outcome. The unit tries to arrange token financial assistance to needy patients from the
     Patients Benevolent Fund of the Institute and also helps in dispatching the medicines etc. to
     the patients.

3.       Research Support and Other Activities

3.1.     Library
The library of the Institute (Category III) has a good collection of books and journals on
different aspects of leprosy and related subjects. In the year 2003, 147 new books were
purchased taking the total number of books to 4007. The additions during the year include
many books on medical subjects as well as important Hindi literature. As in the previous
year, 43 Indian and foreign journals were subscribed during this period. The library has
3543 bound volumes of the journals. More than 500 books have been permanently issued to
departmental sub-libraries.
Besides this, under modernization of ICMR library programme, several softwares have
been provided for betterment of research activities conducted at the Institute: (1) LIBRIS
Software for automation of library ; (2) OVID Medline Abstracting Services on CD-ROM ;         3
(3) ProQuest Searchware for imparting abstracting as well as full text services w.e.f. 1994.
 Internet Connection has been provided for various scientific and administrative work
performed in the library day-to-day.
The Library has been contributing to publication and distribution of Annual Reports, CJIL
& OMD Quarterly Bulletins in English and Hindi, and 'PRERNA'- a Quarterly Hindi
magazine.

3.2.     Biostatistics Unit
During the year 2003 biostatistics unit provided support to different projects

3.3.     Experimental Leprosy Laboratory
Like the early years, this laboratory continued to assist the clinicians in monitoring
therapeutic trials and screening for drug resistant and persister organisms. The laboratory
has the necessary facilities and infrastructure to breed and maintain laboratory animals.
Like previous year, it provided the animal facilities to other scientists working on various
aspects of leprosy.
During the year apart from mice, 10 rabbits were used for experiments. The animals those
were maintained for various laboratory needs were 934 mice, 2 guinea pigs and 15 rabbits.
Further during this year, 120 surplus mice were sold to Aligarh Muslim University, Aligarh
on payment.

      3.4.     Developmental Activities

      3.4.1. Campus Development
      (a) Potable water supply was provided to more areas of the campus.
      (b) Renovation and repair of the Institute building and residential quarters were
          undertaken.
      (c) The construction of a new building complex to house different laboratories and
          scientists is progressing.
      (d) Landscape of the campus continued to improve with green areas expanded.

      3.4.2.   Computer Facilities
      Computer facilities were further extended to include several sections of the Office, OPD
      and Laboratories.

      3.4.3. New Instruments & Facilities
      The Institute has procured several new ultra modern, sophisticated instruments during
      2003-04 which included Biochip Production Unit (Microarray Spotter - Biorobotics) ,
      Hybridization Station (Perkin Elmer), Microarray Scanner (Packard- Perkin Elmer), Protein
      Sequencer (Applied Biosystems), Electrical Incinerator (M/S Aseptic Systems,
      Coimabatore), Liquid Nitrogen Production Unit (Iwatani Corporation, Japan). The newly
      commissioned BSL-3 laboratory for Animal Experiments was further developed.

1.         DBT multicentric collaborative project : The Institute is involved in various
           multicentric collaborative projects on Tuberculosis and other mycobacteria funded by
           Department of Biotechnology. Most of these projects were on `Drug resistance in

      4.3.     Academic interaction with universities and other research
               institutions
      The Institute continued to provide teaching, research and training support for
      M.Sc. courses in Life Sciences (Biotechnology, Biochemistry and Microbiology) of
      Dr. B.R. Ambedkar University, Agra. During 2003 about 120 students have been benefited.
4     Besides a total of 38 students from School of Life Sciences, Dr. B.R. Ambedkar University,
      Agra and 22 students from other universities worked in various on-going projects of the
      Institute for their M.Sc. dissertation. Besides the Institute has also accepted students from
      various universities /institutions as summer trainees and provided them facilities and
      guidance. Facilities were also provided for short term training to the student and faculty
      members from various universities/research institutions.
