Registration and Payment Procedures in Detail
Step 1 – Select Registration Type.
This step has 3 registration types available to the participant. Choose among the following:
1) Online Registration 2) Paper Registration (PDF) 3) Paper Registration (Flyers)

Step 2 – Fill Out Initial Registration. Participant is asked to fill out registration form.
1) ADNEP Website – Fill out Initial Registration
2) Paper Registration (PDF) ‐ Download, print and fill out Paper Registration (PDF) found in ADNEP website
3) Paper Registration (Flyer) ‐ Fill out Paper Registration (Flyer) distributed among schools and health institutions
Step 3 – Pay Convention Fee
Payment made through bank deposit
Go to any branch of BPI Family Savings Bank
(Take note that BPI Bank of the Philippine Islands is different from BPI Family Savings Bank)
Fill out the deposit slip commonly found at the waiting area.

Your deposit slip will be validated, stamped and will be given back to you.

Step 4 – Complete Final Registration
*If you paid directly to PADE/ADNEP Board of Directors, you no longer need to complete the Final Registration.
**If you initially registered online through ADNEP website, click FINAL Registration for August 2010 Convention found on
the right side of the main page. Provide payment details and answer survey.
***For Paper Registration (PDF) or Paper Registration (Flyer), fill in the remaining payment details (branch of deposit, date,
time and amount) found in your validated deposit slip.
Send the accomplished form through fax.

Step 5 – Confirmation of Registration from the Organizers
Participant will receive email and/or text message from the Registration Committee as soon as
payment is confirmed.
Note: Bring the validated deposit slip on August 27, 2010 in exchange of the Official Receipt.
