Identity Theft Prevention Notice
Dear Consumer:
The State of New Mexico is committed to the security of your personal
information and the prevention of identity theft.
Pursuant to the New Mexico Small Loan Act and rules promulgated by the New Mexico
Financial Institutions Division you are currently not eligible to enter into a Payday Loan
because the Date of Birth submitted with your Customer ID is different than
information submitted for a prior Payday Loan by this, or another, licensed lender
in the State of New Mexico.
You will be eligible to apply for a new Payday Loan after your identity has been verified.
To verify your identity and date of birth, the following documentation is required:
1. A certified copy of your birth certificate. A certified paper copy can be obtained from
the Vital Statistics office in the state where you were born.
AND
2. The completed contact portion of this Identity Theft Prevention Notice.
You must mail the above documentation to the New Mexico Payday Loan Customer Care
Center at the following address:

Your will be notified with the status of your request upon receipt of both documents. If
you have any questions about this Notice, please contact the New Mexico Payday Loan

Documents submitted for Date of Birth Corrections will not be returned.
