Attached are Self-Directed Care brochures for distribution to Case Management staff. Please direct staff, including AAA contracted case management staff, to give out the brochure at the time of assessment and reassessment regardless of setting. Current AASA clients receiving InHome IP services have already been sent the brochure. You may order additional copies of the brochure through the DSHS Forms Warehouse under the form number DSHS 22-388. Please use a Forms and Publications Request Form (#DSHS 17011) to request your order and fax it to 360/664-0597, or mail the form to: DSHS Forms Warehouse PO box 45816 Olympia, WA 98504-5816 The brochure is also posted on the DSHS/AASA Internet website at www.aasa.dshs.wa.gov, and may be printed off the website. Translations for the brochure are currently being done and will be posted on the Internet as they are completed. They may also be printed off the website.
