Cover Forms and MAAECF Ethics in Data Collection Forms being submitted for this district
named below.

   I hereby certify that I am aware of the MAAECF Signature Cover Form(s) and MAAECF Ethics in

   Data Collection Forms being submitted with MAAECF portfolios for students in

   _____________________________. I also certify that the teacher, Special Education Director,

   and school principal whose names appear on the MAAECF Signature Cover Form are employees of

   this district. I further certify that each has been informed that falsified data will be considered a

   testing violation and that the Mississippi Department of Education will render the appropriate actions

   that are consistent with testing irregularities as outlined in the Mississippi School Accountability

   Standards, 2009, Appendix F.
