      Campus Service Delivery Plan Instructions
  Campus Planning Overview

  The Campus Service Delivery Plan (SDP) is essential to the implementation of the CIS Campus
  Model. The SDP is a tool designed to assist in developing a comprehensive and detailed plan to
  meet the needs of the students and families that will be served. The SDP outlines the services,
  activities and resources that will be provided on the campus to meet student and campus needs.
  The SDP is developed annually for the school year to include the summer program. A separate
  SDP may be developed for the summer program. In order to ensure that there are planned services
  in place on the first day of school, the assessment and planning process should begin in the spring
  through the summer. This will also ensure that your principal has reviewed and approves of the
  plan so that services can be coordinated and implemented when school starts.

  When establishing a new campus, the planning process must be completed prior to the
  implementation of services. Campus planning is an on-going process linked to the Campus Needs
  Assessment and updated as new needs and services are identified. The SDP will assist you to plan,
  develop and/or broker resources and coordinate services with other providers. The SDP must be
  kept current and reflect the ongoing changing needs of the campus, students and families.

  Each campus must have readily available services that support each of the Six Components
  selected from the state approved Service Code Directory. The SDP will include services to meet
  the needs of case managed students, school events which benefit the entire school population and
  one time services or occasional services for “others”.

  New Campus

  If you are on a new CIS campus, you will be responsible for creating a Campus Service Delivery
                                                                                      d
  Plan with approval from the principal. A thorough needs assessment is crucial to i entify the
  issues addressed by the SDP. Once you have a better understanding of this document, meet with
  the staff to discuss the various components in order to implement an efficient service delivery
  system on the campus.

  Existing Campus

  For existing CIS campuses, review the current or previous year’s plan in preparation for
  developing the new SDP. The planning process should begin before the end of the school year for
  services to being in the next school year. For summer programs, the planning process should
  begin in the spring.

  Campus Profile Sheet

  An important part of campus planning begins by acquiring specific campus information. This
  information will help you to become more familiar with the population that you will serve the
  needs of the campus, and will be useful in planning activities. If school board eligibility criteria is
  adopted attach a copy of the criteria to the profile. The profile information must be entered in
  CISTMS.

  Campus Agreement

  The Campus Agreement is a formal written agreement that addresses essential elements important
  to implementing the program on the campus. Campus Agreements may address other elements
  not listed on the form. Another type of campus agreement may be used at the discretion of the
  principal. It is crucial to have an understanding of the principal’s requirements and approval
  before providing any services on campus. Discussing the items listed will provide the case
  manager with information about the different processes and policies of the campus. Discuss the
  type of information CIS will need for reporting purposes and the best way to obtain that
  information. It is also important to know if the principal expects the case manger to take on duties
  beyond those of CIS. Issues such as hall monitoring, lunch and detention duty can cause some
  concern if the principal expects the case manager to fulfill these roles. By carefully reviewing and
  discussing important topics related to the success of the program, the case manager can avoid
  difficulties of campus management. The Campus Agreement requires annual renewal. Locally
  developed Campus Agreements may be used.

  Campus Service Delivery Plan Approval

  After you have completed your Campus Service Delivery Plan, the local program administrator,
  executive director (ED) or program coordinator (PC) must review it. Please note that the school
  administrator must approve the plan prior to implementation of any activities. Use this sheet to
  obtain signatures from the principal and executive director.

  Key Campus Personnel

  This tool identifies the key campus personnel within the school providing an at-a-glance of the
  campus staff. The key campus staff will assist the case manager in identifying issues and needs to
  address on the campus. List the name of the staff in each of the key roles and their room location.
  This document can also used to set up meeting times with these individuals. Document
  information gathered from these individuals on the Campus Needs Assessment.

  Campus Service Provider List

  The Campus Service Provider List is a tool used to document the service providers and the
  services already available on the campus. Information on this list contributes to identifying
  services that are already available on the campus. Identifying existing services and providers is
  part of the assessment process and helps to avoid duplication of services. CIS may need to
  establish partnerships with the providers in order to coordinate or collaborate services.

  Activity Planner

  The Activity Planner is a tool to assist in planning activities or meetings with campus staff. The
  planner can be used to plan activities such as CIS orientations, CIS open house, marketing of CIS,
  or relationships building. These types of activities may be year round activities that will support
  your efforts in developing services and implementing the campus model.

  Needs Assessment

  In order to develop a comprehensive service delivery plan, it is important to identify the needs of
  the campus and students, the types of services available, and gaps in services. The Needs
  Assessment is designed to provide the case manager with this information so that services can be
  provided to address the identified gaps in service. It documents service needs of students and
  their families as well as policies and procedures that contribute to a positive campus environment.
  The Needs Assessment is conducted before the beginning of the school year and is to be kept
  current as new needs arise. As part of this process, it is important to use the Key Campus
  Personnel sheet in order to meet with the staff that will help you identify the needs and gaps in
  services on the campus. Students, family members and other individuals that may have
  knowledge about the service needs of the campus should be included in the needs assessment
  process. Review each item on the needs assessment, determine if the services are available, and
  needed on the campus. Use the comment section to note any supporting evidence of need.
  Discuss the results with the school principal to determine which areas CIS is to target. This
  information will be used to develop the Six Component activities that will be provided on campus
  to address the identified needs.

  Needs Assessment At-A-Glance/Summary
  Two documents are provided for use in summarizing the identified needs. One document is a
  chart in an at-a-glance format that can summarize the identified needs and the respective
  component. The other document is for a general written summary. Either or both documents may
  be used. Summarizing the Needs Assessment is a helpful process to assist the case manager in
  analyzing all the needs and how to best address those needs.
  Agency Service Provider List

  As the needs assessment process is conducted, needs that are not available on the campus but are
  available in the community will be listed on the Agency Service Provider form. Document the
  contact person and all contact information on this form. This will become a handy reference as
  you identify services in the community that will be available to assist the students and their
  families. Contact with these organizations regarding services is essential in developing a network
  of support for the program. Collaborations may emerge from these contacts and these services
  can become a part of your Six Component Service Delivery Plan.
  Six Components Service Delivery Plan
  The CSDP is designed around the framework of the Six Component Activities, which are central
  to the CIS program service delivery model. CIS Six Component Activities are planned to address
  the issues identified in the Needs Assessment.

      The CIS Six Components are:
      Ÿ Supportive Guidance and Counseling
      Ÿ Health and Human Services
      Ÿ Parental and Family Involvement
      Ÿ Career Awareness and Employment
      Ÿ Enrichment
      Ÿ Educational Enhancement
  Six Component Activities are developed to bridge the gaps in the school’s current delivery system
  and to address the identified needs of the students and campus. It is important to recognize that
  service delivery plan is an on- going process. Once the targeted areas have been identified, the
  case manager can begin to develop activities to address those needs. Each campus should have
  readily available services from each of the CIS Six Components selected from the state approved
  Service Code Directory.

  The Service Code Directory (SCD) is a list of state approved activities categorized under each of
  the six components. The SCD is used in campus planning and service delivery. For consistency,
  each activity is assigned a specific service code for tracking and reporting purposes. On-going
  services are planned to meet the needs of case managed students as well as one time or occasional
  services for “others” and all school events to benefit the entire school population.

  Instructions for the Six Component Plans

  Activity Name & Description: In this column, list all of the activities under this component that
  you plan to offer on your campus. When listing an activity or event, give brief and definite, but
  adequate information to make it clear what the activity or event involves. If, after planning an
  activity, you see later that it is not feasible, mark out the activity and write in your alternative
  activity. Knowledge of specific student needs, available resources and staff creativity are key
  ingredients to utilize when planning activities. If an activity addresses more than one component,
  show each part of the activity under the appropriate component. It must be documented as a
  distinct activity even though it occurs in conjunction with another activity. Activities that are
  planned must be implemented or another activity must be provided in its place. CIS addresses
  some needs such as clothing and school supplies. If these types of resource services are the only
  ones listed under a particular component, the component is not adequately in place. Local
  programs may establish a minimum number of activities under each of the six components. If
  more than one sheet per component is needed, duplicate the master sheets.


  Date & Time : List dates and times for your activities. If these times and dates are still pending
  at the time you write your plan, list your ideal projected time and date. If the projected time and
  date change, insert the new information as soon as possible. Avoid using “TBA” and “pending”.
  Use “on-going” or equivalent only when absolutely necessary and appropriate.

  Provider: List the name and group (CIS, other agency/organization as applicable) of the person
  primarily responsible for this activity. If a number of CIS staff are to be involved, list the primary
  person’s name and indicate “and CIS staff”.

  Location: Give actual room numbers, campus areas, and/or other specific sites where you plan
  for the activity to take place. Activity location needs to be much more specific and definitive than
  just listing the campus or school name. Again, avoid “TBA” or “pending”.

  Resources: List any resources you will need to carry out this activity.

  Date Completed: Once the activity has been completed, document on the form.


  Budget

  The budget assists the case manager in developing a budget to meet the proposed service delivery
  plan. The program or campus may provide needed funding for the activities. If not, the case
  manager will be informed and can plan fund raising activities accordingly to meet the service
  delivery needs.

  Calendar

  The calendar is a tool to list the activities for each month. Post the calendar in the office and share
  with campus staff, students, parents and service providers to keep them informed of CIS activities
  taking place on the campus.

  Campus Service Delivery At-A-Glance

  This form provides the case manager with a snapshot of all the planned services for each
  component. The Service Delivery At-A-Glance if checked against the Needs Assessment At-A-
  Glance will help meet all the identified needs.

  Summer Plans

  The summer program ensures the continuity of services for case managed students in need of CIS
  services beyond the end of the regular school year. Due to the variety and style of the summer
  programs, prepare an additional SDP for the summer session. The planning process should begin
  in the spring. Include feedback from the principal when developing the plan and ensure that this
  plan supports the campus summer program needs. A new needs assessment is not required, and it
  is not necessary for all six components to be in place. The duration of the summer program is
  determined locally.

Key Points

  Ÿ The services must reflect the campus needs assessment and address the needs of the students
      and families.
  Ÿ A well-planned program will contain a variety of activities and events for each of the six CIS
      components.
  Ÿ   It is important to write your plan in a clear and detailed manner, so that it is understandable to
      others.
  Ÿ   Refer to the Service Code Directory when planning activities.
  Ÿ   The campus plan should include services provided to meet the needs of the entire campus
      through school-wide events and after-school activities.
  Ÿ   Include services provided by other service providers that have a written service agreement.
  Ÿ   Plan a variety and number of activities and tasks under each component. One activity under
      each component is not adequate.
  Ÿ   Planned activities are not to duplicate existing services, but rather to bridge the gaps in the
      school’s current delivery system.
  Ÿ   The SDP must reflect all activities that CIS provides including those to meet the Campus
      Improvement Plan requirements.
  Ÿ   List all planned activities on the SDP with the corresponding six components using the
      Service Code Directory as a guide. Do not list a planned activity on more than one
      corresponding component sheet.
