Each attendee must fill out a separate form unless buying a table. If you are buying a table we ask that you supply us
names prior to the dinner. If your company is paying for multiple registrations (but not a table) then each attendee
must complete a registration form and the check should be enclosed with all of the forms. Should it be impossible
for a company check to be attached at the time of registration it is the responsibility of the individual attending to
ensure payment prior to the dinner.

Deadline for reservations is 8pm Friday May 16th. Registrants will be charged an additional $5 late fee for
any registrations received after that date. No e-mail registrations will be accepted under any circumstances.


Directions: I-91 to Exit 12, head north on Route 5 (Washington Avenue). Fantasia is on the right at 404 Washington
Avenue, next to Connex Credit Union and opposite Pratt & Whitney.
