As evidence of my/our desire to provide a legacy of support to the American Association of 
Swine Veterinarians (AASV) Foundation, I/we hereby inform the AASV Foundation that I/we 
have made a provision for a gift to the Foundation in my/our estate plans. I/we understand that 
this commitment is revocable and can be modified by me/us at any time.

                Feel free to publish my/our name(s) among your lists of Heritage Fellowship 
        members as a motivation for others to leave a future gift to benefit the AASV 
        Foundation. (The amount of your gift is not published.) 
        Please list my/our name(s) internally to the AASV Foundation only (no outside 
        publication). 
        Do not list my/our names either internally or externally (anonymous gift).
