The supplier may issue this declaration on the request of the buyer, who uses it as evidence when he applies for
a certificate of origin for the exportation of the goods.
 It should only be used within the European Community.
 The text in the frame below, suitably completed in conformity with the footnotes, constitutes the declaration
of origin. The footnotes do not have to be reproduced.
 This declaration is to be placed on the supplier's letter paper.

This declaration is not valid for goods having preferential origin status and which qualify for move-
ment certificates EUR.1.
