"Confined Space Job Hazard Analysis Guide"
Confined Space Job Hazard Analysis Guide Occupational Health & Safety Program Northwood Pulp Mill Page 1 of 3 CONFINED SPACE - JOB HAZARD ANALYSIS – GUIDELINE The JHA is the means by which all WCB regulatory requirements are to be addressed. It is important to understand the regulations an refer to them if necessary when competing the JHA. Confined Space/Vessel: Equipment No. Date: Preparation date Work Order Expiry Anticipated completion date is Required No. Date: Job Contact Reviewed Have the JHA reviewed by another person who By: has experience in completing them, (i.e. planner, (prepared by): supervisor, engineer) Refer to the WCB Regulation and utilize OH&S / IH resources if you unsure about any of the requirements. Description of Work & Authorized Entrants Break down to main work activities and identify the group or crew who will be conducting each activity. e.g. Staging - general trades Welding/grinding – welders Removal of bearing – millwrights Clearly identify Restricted Entry Activities if applicable. Work Generated List all potential work generated emissions and relate to the applicable activity shown Emissions above. e.g. metal fumes and gases from welding. Dusts from grinding. Atmospheric low / moderate / high as per WCB definitions. Hazard Low – no measurable contaminants. (i.e. ventilation, or respirator) High – conditions which may result in serious injury, illness, death or may otherwise impair workers ability to escape unaided in the event of a control system failure Note: Rescue pre-plan required for high risk activity, See the Protection Office Additional Identify contaminants that require monitoring. Include the frequency of monitoring Monitoring required and specific action and ceiling levels that apply. Not all potential contaminants can be monitored directly. (The monitoring frequency will be dependent on the pattern of work activities. Action levels for Restricted entry should be based on maximum use concentrations of the respiratory equipment in use or applicable IDLH values Continuous monitoring of selected contaminants required for restricted and high risk entry) e.g. Carbon monoxide - every 1hr. during welding activities, action level 12.5 ppm (based on 50% of the WCB exposure limit) Additional Specify the volume (cfm) required, the equipment to be used and placement of the Ventilation required equipment for effective ventilation of the space. (If contaminant levels are not within the specified action levels during the work, the ventilation should be altered and alteration recorded on the JHA) Other Hazards List any additional hazards and applicable precautions. related to work & Hazard may be related to the work in or around the space. Precautions e.g. Wet environment - GFI panel required. Corrosive chemical handling – Supplementary eye wash and shower equip. Work above 10' - specific fall protection equip or reference plan Fire hazard - fire extinguishers / hot work permit / non-sparking tools Additional P.P.E. Specify type of PPE (gloves, clothing, eye protection, respirators) required and which required activities it applies. • (e.g. Dust mask to be worn during all grinding activities)). Include Maximum use concentrations if applicable. PPE requirements are identified from MSDS, Dept./Mill Procedures, WCB regulations, OH&S Dept.
