Service agreement Name, address, phone # Social security #, date of birth Driver’s license or state ID (must verify) Signature Dealer Code Customer’s SSN 15 digit IMEI (handset serial number) 19 digit SIM number Call Customer Care (or use WATSON) and provide customer’s SSN If the account is less than 60 days old, Customer Care will pull original ONYX score If the account is older than 60 days, Customer Care will look at payment history before approving an add-on
