HEALTH SCIENCE PREREQUISITES

The prerequisite courses you need to complete prior to admission were noted in the materials we mailed to you
with your acceptance letter. Please refer to those materials to be sure you know all remaining course
requirements. (Please see the next section for general education, Gordon Rule and CLASR requirements.)

If you are interested in taking summer electives, the list below includes examples of courses our students have
selected in the past. You are under no obligation to enroll in any of the courses listed below. Please view the
Schedule of Courses for additional course options if you are considering taking summer electives.

Any student that does not have their professional prerequisites done must take those courses during Summer C.

GENERAL EDUCATION, CLASR, AND GORDON RULE

All students, including transfer students from private or out-of-state universities, must complete general
education courses comparable to those required of students at the University of Florida. You must also meet
CLASR and Gordon Rule requirements for math and writing, as described below. All requirements must be
completed prior to admission to the limited access BHS program as specified in the application materials.

   The College Level Academic Skills Test (CLASR) http://www.registrar.ufl.edu/currents/clastinfo.html

       o The State of Florida requires that all students demonstrate competency of basic college
         communications and mathematics skills. The College Level Academic Skills Requirement
         measures achievement in communication and computation skills in four categories: essay,
         English language skills, reading and mathematics. The College of Public Health and Health
         Professions requires that students complete the requirements before entering the junior year of the
         BHS program.

       o Students can satisfy the CLASR through approved course work (six credit hours with a 2.5 GPA),
         or qualifying test scores from SAT or ACT exams.

       o Please contact a PHHP advisor at 352-273-6400 if you have questions about whether you have
         met the CLASR requirements through your coursework

   Gordon Rule Requirements

           Math
       o   You must complete a minimum of 6 credits of college level, transferable credit
       o   The courses must be at or above the level of college algebra
       o   You must have a grade of C or higher for the course to count towards the math requirement
       o   At least one course must be in mathematics; the other can be in math, statistics, computer
           science, or logic

           Writing
       o   You must complete a minimum of 24,000 graded, written words
       o   The course syllabus must state the number of written, graded words required in the class
       o   You must have a grade of C or higher for the course to count towards the writing requirement
       o   No course can be credited with greater than 6,000 words

IMMUNIZATION REQUIREMENTS (see checklist for dates)


A. Students Currently at UF: (Please retain copies for your records)

Varicella and Other Immunizations - If the original immunization form you turned into the Student Health Care
Center (SHCC) did not include documentation of Varicella vaccination (Chicken Pox), you can satisfy the
Varicella vaccination (Chicken Pox) immunity requirement by providing documentation of one of the following
to the SHCC on their Http://shcc.ufl.edu/medical/pdf/acl-002a.pdf

     i.      Certification of a Varicella Vaccination or a positive titer from a health care provider documented
             on the Http://shcc.ufl.edu/medical/pdf/acl-002a.pdf

     ii.     Medical documentation of having been treated for Chicken Pox by a physician documented on the
             Http://shcc.ufl.edu/medical/pdf/acl-002a.pdf

You must also ensure that all of your immunizations are up-to-date. Your admission to the junior year in
PHHP is contingent upon all required medical documents being on file with the SHCC by the deadline.
Your admission will be rescinded if you do not meet the deadline. For additional information about
immunizations, see the Student Health Care Center website at http://shcc.ufl.edu/.

Tuberculosis (TB) Skin Test - A negative TB Skin Test (or chest x-ray if the TB test is positive) taken after
5/1/10. Both Shands Student Health Care and the SHCC on Fletcher Drive administer the TB Skin Test every
weekday but Thursday. If you receive a TB Skin Test through Shands Volunteer Services, you must ask for a
printout from them to provide to the SHCC. See checklist for deadlines. The TB Skin Test is required each
year of the program and must be documented on the Http://shcc.ufl.edu/medical/pdf/acl-002a.pdf . If you have a
TB Skin Test administered by an outside medical facility, you must take the SHCC mandatory form with you for
the results to be documented on.

B. Students Transferring to UF: (Please retain copies for your records)

In addition to the University immunization requirements, the College requires students to submit
documentation for two other immunizations: Varicella vaccination (Chicken Pox) and Tuberculosis (TB)
Skin Test.

GATORLINK E-MAIL ADDRESS AND COMPUTER REQUIREMENTS

 You must establish a GatorLink e-mail account with an address ending in ufl.edu (e.g. shanson@ufl.edu).
 You will then include this e-mail address on the Contact Information Sheet. You must have access to a
 computer that houses this UF e-mail account. Mail can not be forwarded through another e-mail system.

 Although you are not required to purchase a computer, you must have access to a recent version of
 PowerPoint and Microsoft Word software and are responsible for knowing how to use the Internet, e-mail,
 PowerPoint and Microsoft Word software. You may acquire computer access through the most
 appropriate option to match your individual circumstances. These include, but are not limited to, computer
 purchase, lease, loan, etc. If you lack knowledge in the computer systems and software packages required,
 you must acquire this knowledge prior to your admission term. Community education or self-study
 programs may be avenues to acquire this knowledge.



UNIVERSITY OF FLORIDA IDENTIFICATION BADGE

   You must have a Gator 1 Identification Badge to be able to access specific course materials and university
services and facilities. Please make sure your Gator 1 Badge indicates that you are part of the Health
Science Center. **If you are a UF student and your current Gator 1 Badge does not have Health Science
Center noted on it, you must replace it.** The current cost is $15.00 and $1.25 for a plastic holder.

The College of Public Health and Health Professions has scheduled a specific time for Bachelor of Health
Science juniors to have their photos taken and Gator 1 Badges made. Please be prompt, those are the only
hours set aside specifically for you:

Location: Gator 1 Central, UF ID Card Office, Communicore Building, Room C3-303
Date: Friday, August 20, 2010 (BHS Orientation Day)
Time: 9:00 AM – 12:00 Noon
Cost: $15.00 for the ID Badge and $1.25 for the plastic holder
Payment method: Cash, check, Visa, AmEx, MasterCard, Discover, or debit card
Identifications Required: Acceptance letter to the BHS program, a pictured ID such as a driver’s license or
passport and know the last 4 digits of your Social Security Number.

Completion of HIPAA Requirements

WHAT IS HIPAA?

HIPAA is the Health Insurance Portability and Accountability Act of 1996. It is a federal law that helps
protect patient confidentiality. All students in the College of Public Health and Health Professions are
required by this law to demonstrate a basic understanding of HIPAA.

WHAT DO I NEED TO DO?

All students need to complete on-line HIPAA training. In order to complete the training, follow the steps
below:

           o Go to the HIPAA website http://privacy.health.ufl.edu under Training

                     Complete the on-line HIPAA course (allow about one-half hour of concentrated time
                      for the test given it will time out if not finished in one sitting)
                     Print the Certificate of Completion
                     Print, read and sign the Confidentiality Statement at the same web address

WHEN SHOULD I COMPLETE THE HIPAA TRAINING?

           o Any time after you have a UFID number and before the deadline date on your checklist.

WHERE DO I TURN IN MY CERTIFICATE AND CONFIDENTIALITY STATEMENT?

           o Turn in the HIPAA Certificate and Confidentiality Statement along with your other
             required forms to the Dean’s Office (4th floor HPNP Bldg. room 4101) by the deadline on
             your checklist. Retain copies for your records.

                     Your admission to the College will be revoked if your HIPAA Certificate and
                      Confidentiality Statement are not received in the Dean’s Office by the deadline.
                      These items are required each year of the program.

WHAT IF I HAVE QUESTIONS?

Please note: Even if you completed HIPAA training at another institution, including Shands Hospital, you
must repeat the on-line training and sign another Confidentiality Statement by the deadline.

B. Completion of Bloodborne Pathogens Training

All students must complete the Bloodborne Pathogens (BBP) training by the deadline date on your
checklist. The training is located on line at http://bahealthsci.phhp.ufl.edu/bbp/ The BBP training is
required each year of the program.

The HIPAA Certificate, Confidentiality Statement and Bloodborne Pathogens Certificate are to be turned
in with your final transcript and Contact Information Sheet. All forms must be turned in at the same time.
Please review the checklist for the dates.



CONTACT INFORMATION SHEET

Print out, complete, and provide us with your Contact Information Sheet. Please make sure you include
your name, work and home phone numbers, GatorLink e-mail, and address. ONLY the GatorLink e-mail
will be used for correspondence regarding the BHS program and University matters of potential interest.
The Contact Information Sheet should be turned into the Dean’s Office with your other required forms by
6/15/10 for summer admission and by 8/10/10 for fall admission. We will keep this information in the
Dean’s Office and will access it in the unlikely event that we cannot reach you and are concerned about
your safety or welfare. Please note: the college Contact Information Sheet is in addition to the emergency
contact information required on ISIS by UF.
