Description

This 2-year grant will help fund a number of model
programs addressing physical acitivty and nutrion under
the Healthy and Active Communities Initiative.
This 2 -year grant will provide physical education and after
school programs that will help youth and families live
healthier lives in the communith through local policy
changes under the Healthy and Active Communities
Initiative.
This 3-year grant will help to institute a community-wide
comprehensive fitness and nutrition education program
under the Healthy and Active Communities Initiative.
This 3-year grant will address physical activity, health
eating, and lifestyle changes for the residents of Carter,
Iron, Reynolds, Shannon and Wayne counties under the
Healthy and Active Communities Initiative.
This 2-year grant will fund an after school program that
provides organized soccer, regular physical activity and
nutrition education to urban children under the Health and
Active Communities Initiative.
This 3-year grant will help to teach mature adults how to
be physically active under the Healthy and Active
Communities Initiative.
This 2-year grant is to implement its Hip Hop Health
program, which provides nutrition and physical education
program to school aged children under the Health and
Active Communities Initiative.
This 2-year grant would implement a media literacy and
advocacy program designed to produce healthy behavior
in students and the community under the Health and
Active Communities Initiative.
This 2-year grant will provide funds for its nutrition and
fitness programs, which focus on reaching children
through schools, recreation centers and churches under
the Health and Active Communities Initiative.
This 3-year grants supports a variety of community-
focused activities in south-central Missouri to promote
nutrition and physical activity through special events,
education, and worksite wellness evaluations.
This e-year grant will help to expand the YMCA Activate
American national program under the Healthy and Active
Communities Initiative.

This 2-year grant will provide Vitality, Vim, and Vigor to lay
health educators interacting with and educating the
minority community on physical activity, nutrition and
obesity prevention unnder the Healthy and Active
Communities Initiative.

This 2-year grant will expand physical activity, nutrition
education, and policy implementation in a 5-county area
under the healthy and Active Communities Initiative.
This grant provides for evaluation of programs funded
through MFH's Healthy & Active Communities Initiative.
This 2-year grant supports sports league memberships for
children, worksite wellness programs for adults, school
wellness education programs, and healthy behavior clsses
for preschools and WIC offices, as part of MFH's Healthy
& Active Communities Initiative.

Funding enables the health department to develop
worksite, church-based and after-school wellness
programs, and especially targeting low-income, Aftrican-
Americans, women and children and families. This is part
of MFH's Healthy & Active Communities Initiative.
In Wayne County, the district is using this 2-year grant to
support its 'Tiger Fitness Challenge,' which includes
constructin of a student fitness center, assessment of
student fitness levels and nutrition education for almost
1,200 students age 5-18.
With this 2-year funding, this central Missouri school
district is implementing its 'Improving Community Activity
and Nutrition ' (ICAN) program to address obesity
prevention by strengthening public policy and increasing
access to physical activity opportunities and healthy foods
in the schools and community.
For residents in south central Missouri, this 2-year grant
through MFH's Healthy & Active Communities Initiative
funds school-based nutrition and physical activity
programs, construction of walking trails, worksite wellness
programs in the schools, and a community-wide
awareness campaign.
Thrugh MFH's Healthy & Active Communities Initiative,
this 2-year grant funds a partnership with the Clark County
Health Department to increase awareness of nutrition and
healthy behaviors, develop physical activity challenges
and education programs for families, and provide
scholarships to fitness facilties.
This 2-year grant supports the 'Fit For Life' program, which
focuses on nutrition-related interactive classes and
seminars at school, field trips and family- and staff-friendly
physical activity classes.
This 2-year funding supports the organization's Active
Living Policy Initiative, which is designed to develop and
implement a toolkit for obesity-prevention policies. The
project includes needs assessments in four St. Louis-area
communities and a report to the community.
This 2-year grant funds a 16-week, after-school
nutrition/fitness curriculum based on BJC's Fun-tastic
Nutrition program in St. Louis-area public and private
schools. This is part of MFH's Healthy & Active
Communities Initiative.
Through a 2-year grant as part of MFH's Healthy and
Active Communities Initiative, the club is focusing
youngsters on behaviors that promote healthy eating,
increase physical activity and encourage personal goal
setting. A weight management program is in collaboration
with St. Louis Children's Hospital.
This 2-year grant funds the food pantry's efforts to provide
clients with better access to healthier food choices. As
part of MFH's Healthy and Active Communities Initiative,
the program includes nutrition education, hands-on
cooking demonstrations, physcial activity education,
organized walking clubs and links to fresh fruits and
vegetables.
This program focuses is on walking for recreation and
transportation for older adults. Components include
developing walking kits, maps, step programs and the
consumption of healthy foods. The 2-year grant is part of
MFH's Healthy and Active Communities Initiative.

This 2-year grant funds implementation of the Healthy
Schools program, a nationwide effort to encourage active
living and healthy eating. In partnership with nine school
districts throughout Missouri, the program develops
wellness policies and staff wellness programs, and works
to improve vending machine choices. The grant is part of
MFH's Healthy and Active Communities Initiative.
As part of MFH's Healthy and Active Communities
Initiative, this funding enables the organization to offer
obesity prevention activities and programs, train area
mental health professionals about obesity prevention, and
implement worksite policies to promote active lifestyles
and healthy eating.
This 2-year grant, as part of MFH's Healthy & Active
Communities Initiative, supports the start-up of a farmer's
market in north St. Louis that offers healthy food, along
with free nutrition education, cooking demonstrations,
health screenings and bicycles to local residents without
transportation.

The 'Get Healthy and Active Desoto' program is in
collaboration with the DeSoto School District to implement
school-based programs to improve eating habits and
physical activity for 3rd graders and school district staff.
The 2-year grant, part of MFH's Healthy & Active
Communities Initiative, also supports development of a
county-wide physical activity and nutrition coalition.
In partnership with Barnes-Jewish St. Peters Hospital, the
'Walk & Roll' program focuses on increasing physical
activity for adults primarily through worksite programs.
This 2-year grant is part of MFH's Healthy & Active
Communities Initiative.

As part of MFH's Healthy & Active Communities Initiative,
this 2-year grant funds continuation of the Garden of Eden
program, a collaborative effort with African-American
churches and community partners to provide health
advisors, nutrition education, exercise classes and the
purchase of fresh fruits and vegetables.
This grant is part of MFH's Healthy & Active Communities
Initiative, and provides for evaluation of programs
implemented by initiative grantees, as well as evaluation
of MFH's effort to tackle obesity in Missouri through this
funding program.
Funded through MFH's Healthy & Active Communities
program, this grant funds expanded efforts of the south
central Missouri collaborative to improve nutrition
education, physical activity, healthy policies in schools and
worksites.

Initiative, this grant supports the 'Fit Phelps - 5:30
Campaign,' which is working on the sustainability and
replication of the organization's 'Fit Phelps' program,
which began in 2005. Activities include after-school
programming, community gardens, a social marketing
campaign, and an emphasis on environmental and policy
changes.
Funded through MFH's Healthy & Active Communities
Initiative, this grant further develops the organization's
efforts to blend physical activity and nutrition education for
St. Louis-area children ages 8-11.
Funded through MFH's Healthy & Active Communities
Initiative, this grant expands the 'Active and Healthy Scott
County Central' school-based project. The goal is to
increase opportunities for physical fitness and access to
healthier food, as well as literacy about the topic, in rural
schools across the Bootheel.
Funded through MFH's Healthy & Active Communities
Initiative, this grant funds replication of the 'Hip Hop for
Health' program in three new St. Louis-area middle
school. The program takes the place of one physical
education class per week and provides a color coding
system in the lunch room to help students identify
healthier food choices.
Funded through MFH's Healthy & Active Communities
Initiative, this grant supports efforts to sustain physical
activity and nutrition behavior changes in older adults
living in the St. Louis area. The 'Healthy Life Project'
includes training additional volunteer staff to reach out to
more low-income and ethnically diverse older adults, to
ensure sustainability and replication.
Funded through MFH's Healthy & Active Communities
program, this grant builds on the organization's 'Vitality,
Vim and Vigor for Life=3Vs' program. The focus is on
obesity prevention programming, comprehensive
evaluation, coordinated program dissemination and
leadership development to sustain the program. The effort
targets the Hispanic/Latino population in southwest
Missouri.
Funded through MFH's Healthy & Active Communities
Initiative, this grant helps refine and test the Springfield-
area YMCA's model program, 'Healthy Kids Academy.'
Funding supports further development of program
activities, addressing challenges found and developing the
academy into a program that can be replicated in similar
school-age settings.
Funded through MFH's Healthy & Active Communities
Initiative, this grant expands the efforts of 18 communities
in five southwest Missouri counties to promote healthy
eating, physical activity and development of policies.
Along with increasing the number of sites for residents to
enjoy healthy activities, and helping communities develop
policies that support those efforts, the goal is to create a
program that can be replicated in other similar
communities.


As part of MFH's Healthy and Active Communities
Initiative, this grant funds development of a database of
existing policies in Missouri communities that support
obesity prevention, and healthy activites and nutrition.

area African Americans, women, children and low-income
families in their efforrts to tackle obesity and improvie their

This 3-year grant funds development of a skate park to
help Poplar Bluff youth increase their physical activity.
Funding comes through MFH's Healthy & Active
Communities Initiative.

With this 3-year grant, the district can provide improved
fitness efforts for elementary students and adults in three
mid-Missouri rural school communities. Funding comes
through MFH's Healthy & Active Communities Initiative.
As part of MFH's Healthy & Active Communities Initiative,
this 3-year grant funds a community awareness campaign,
after-school nutrition education and physical activity
programs, summer walking program, walking trail
enhancements, and school and business worksite
wellness programs.

Healthy Eating with Local Produce (HELP) Project:
Funded through MFH's Healthy & Active Communities
Initiative, this 3-year grant funds the 'Healthy Eating with
Local Produce' project in the Maplewood-Richmond
Heights school district. Kitchen staff are using the produce
from the school gardens in their meals; teachers are
incorporating nutrition and exercise programs into the
health, science and physical education courses.

The 3-year grant supports the development of community
obesity prevention plans that focus on area school and
community environments and wellness policies.

With this 3-year grant, the organization is supporting a
social marketing campaign, through the Ten Toes Express
program, to increase walking in senior in St. Louis region.
With this 3-year grant, the organization can further
develop its weekly farmer's market, by recruting more
vendors and adding children's activities to encourage
community participation.

This 3-year grant enables the organization to train 80 after-
school educators to provide fitness activities to St. Louis
area schoolchildren. Funding comes through MFH's
Healthy & Active Communities Initiative.

Program, to create healthier school environments that
promote physical activity and healthy eating for students

This 18-month grant funds research and focus groups to
help increase health lifestyle awareness in low-income
families living in the Bootheel. Funding comes through
MFH's Healthy & Active Communities Initiative.
In 2009, the Bootheel health department is using the
funding to develop focus groups in order to help determine
obesity prevention needs in the community. The grant is
made through MFH's Healthy & Active communities
Initiative.

Over an 18-month period, funding supports obesity
awareness for Bootheel-area children by cunducting
research among parents and students. The grant is made
through MFH's Healthy & Active Communities Initiative.

supports actions to reduce disparities by increasing
access to healthy eating and physical activities choices for

With this 3-year grant, the organization is expanding

coordinator, working with churches to establish wellness
and fitness programs in their congregations, and
developing community exercise facilities. Funding comes
through MFH's Healthy & Active Communities Initiative.
This 3-year grant supports efforts to adopt a Complete
Streets policy for the county and increase residents'
physical activity through bike routes, safety courses,
community events and a bike-to-school program. The
funding comes through MFH's Healthy & Active

increasing residents' access to local fresh fruits and
vegetables, and promoting healthy lifestyle education. The
