    Department of Defense guidelines for TRICARE Prime enrollments and
    primary care manager assignments
•   If you are being, or have been, assigned to a primary care manager that you
    would like to change; or, if you have been advised by the Health Net customer
    service line (1-877-TRICARE) to request a “waiver” from a TRICARE Service
    Center or Health Benefit Advisor, please note:
       •   Neither the TRICARE Service Center nor Health Benefit Advisor staff
           can issue a “waiver”
       •   Primary care managers (military clinics or civilian providers) cannot
           authorize a “waiver”
•   Naval Medical Center Portsmouth has a grievance process (instead of a
    “waiver request” process) for enrollment issues.
       •   Visit the NMCP Health Benefit Advisors (Bldg 3, 3rd floor) or Customer
           Services (Bldg 3, 4th floor) to submit a grievance
       •   Your request must be in writing, and the staff will assist you with
           preparing the necessary forms
       •   The review process may take several days, and all facts will be
           considered before a decision is made
       •   You will be notified once the decision is made
•   Verify your enrollment and primary care manager assignment before
    scheduling your first appointment
       •   Enrollment capacity at a military treatment facility may change in the
           days between the time a request is submitted and when it is processed,
           so there is a possibility you may be assigned to a primary care
           manager other than the one you requested
       •   To verify enrollment, call the TRICARE Service Center or Health
           Benefits Office 5 to 7 days after the form is submitted
