                      What you need with to bring with you:


   □ Proof of Identification (a photo ID)

   □ Social Security Cards (or Social Security Number verification letters
     issued by the Social Security Administration) for you, your spouse, and
     your dependents,

   □ Birth dates for you, your spouse, and your dependents

   □ Current year’s tax package if your received one in the mail

   □ Wage and earning statements (Form W-2, W-2G, 1099-R) from all
     employers

   □ All interest and dividend statements from banks (Forms 1099)

   □ A copy of last year’s Federal and State returns if available

   □ Bank Routing Numbers and Account Numbers if you want your refund
     deposited in your bank account.

   □ Other relevant information about income and expenses
        o Total Amount Paid for Child Care
        o Child Care providers Identifying number

   □ The attached Intake/Interview Sheet with first side completed (additional
     Intake forms will be available at the tax preparation site)

   □ To file taxes electronically on a married filing joint tax return, both spouses
     must be present to sign the required forms

the tax preparation site.
