I like that "Employee Addiction Assessment" recognizes a very real problem. There are thousands of people in the United States that suffer from addiction and do not know that they have a problem or are to afraid to reach out for help. I agree with the previous comment, I would give this assessment to everyone at the same time so that no one feels personally attacked, and I would somehow make it so that the results were completely confidential to encourage honesty.

What I liked most about this document was that it is a non-confrotational approach to issues that most people can't talk about openly, but spend most of their work day thinking about and being distracted by. It also offers a easy,approachable offer of help that is there when needed. I would use this document in a work environment that is stressfull and present it to all employees at the same time to let them know that no one person is targeted and that the offer is open to anyone.
