   RE:            PROTOCAL/PROCEDURES FOR INSURANCE CLAIMS

   SUBJECT: CLAIM PROCEDURE – LIABILITY

   In the event of any type of loss on school property that causes bodily harm or property damage to
   another person or property (excluding injury to employees), the attached “General Liability
   Notice of Occurrence/Claim” should be submitted with highlighted areas completed. If an
   accident involves a student, please note that the school does not carry medical insurance
   and the student should be referred to their personal insurance, i.e., student insurance or
   personal medical insurance. Please attach a copy of the signed waiver designating
   insurance coverage. Attach all pertinent supplemental information as noted below and send to
   TDO attn: Cindy Zimmerman ext 25.


                    • Police report (if not available get the report # and reporting officer)
                    • Complete description and statements from witnesses
                    • Photos
                    • Contact person and phone number, etc.
                    • Completed claim form with highlighted information required


   A supply of forms is attached. Refer to the Protocal/Procedures for Accidents letter that is
