Instructions to Supervisors and Managers for Reporting Work-Related Injuries/Illnesses
Read these instructions before doing anything else. 1. If the employee is taken immediately to a hospital, call 3250866 as quickly as possible! 2. When an injury or illness is reported to you, the employee must immediately complete the “OU Employee’s Report of On-theJob Injury/Illness.”
State and federal laws require that employees report all injuries that occur at work. OU requires that employees immediately report injuries and that the report is in writing.
5. Complete and sign the “Form 2: Employer’s First Notice of Injury” and the “OU Supervisor’s Report of On-the-Job Injury/Illness.” Fax the Employee’s Report, the Form 2, and the Supervisor’s Report to Risk Management at 325-7238. A “Fax Cover Sheet” is provided for your convenience.
After you learn of an employee’s injury, you have a 24-hour time limit for faxing the completed forms to 3257238.
8. The doctor should provide the employee with “Return to Work Status Report.” The employee must give a copy of this form to his/her supervisor. The supervisor must immediately fax a copy to 325-7238.
It is vital that we know whether the employee is able to work or has been taken off work by the doctor. Call 325-0866 if the employee does not provide the return to work orders or does not show up for work when released by the doctor to return.
3. Read the completed Employee’s Report and have the employee show you where and how the incident occurred.
OU supervisors are responsible for investigating how work injuries occur.
6. If the employee has been injured by a medical device, such as a hypodermic needle or scalpel, complete the “OU Supervisor’s Supplemental Report for Needlestick and Sharps Injuries.” Fax this report with the others.
SEND injuries from contaminated needles/scalpels TO GODDARD OR THE HOSPITAL IMMEDIATELY!!!!
9. Any hours missed from work must be reported as “OJI Hours” on the OU Payroll System.
OU Payroll Division and the Workers’ Compensation Administrator coordinate the very complicated process affecting an employee’s income when he/she cannot work because of doctor’s orders. The only responsibility a supervisor or manager has for this process is ensuring that an employee’s OJI hours are reported on the OU Payroll.
4. If the employee needs to see a doctor, fill out and sign the “Workers’ Compensation Referral” form. Give it to the employee. The employee should provide this form to the approved doctor prior to treatment!

7. If the completed reports cannot be faxed to 5-7238 within 24 hours, please call 325-0866 to explain why.
Until the completed reports are received at Risk Management, the injury is in doubt. A delay in faxing these reports can cause a delay in referrals to surgeons and specialists in extreme cases.
