                                      Tuition Payment Plan
12 monthly payments:

• First tuition payment is due on June 1, 2009. This payment is non-refundable.
• Remaining 11 payments are payable and due the first of each month beginning July 1, 2009
    through May 1, 2010.

Having agreed to the Tuition Payment Plan above, I clearly understand that:

    1. No student will be admitted to school at the beginning of the school year until the
previous year’s financial obligations have been met.

    2. No transcripts, report cards, progress reports, or diplomas, etc. will be issued if tuition
and outstanding fees are in arrears.

    3. Tuition and fees are due on the first day of the month. Payments received after the 10th
will be assessed a $25.00 late charge. If tuition and fees are not paid in full by the 25th
of each month, an additional $25.00 late fee will be assessed.
     4. Fees: (See Registration, Tuition and Fees chart)
Yearbook Fee $ 18.00 per family (one copy)
                    Service Hours Requirement $450.00
(Service Hour fee of $15 per hour is due if 30 hours of service have not
been performed by April 30, 2010) (Ten of the 30 service hours must be
                              performed in fundraising activities.)

     5. This agreement must be completed in full and returned to the Business Office with
registration to complete the enrollment requirements.

     6. Families applying for financial assistance must comply with the above time frame. If
assistance is granted, an amendment to this agreement will be issued.

     7. Supporting Parishioner status for the 2009-2010 school year will be determined by the St.
Rita Church Office. A family is considered in support of St. Rita Church through
donations of a minimum of $15.00 weekly using envelopes between July 1, 2008 and
June 30, 2009. If you are unsure of your status, please contact the church office for
clarification. Supporting status for tuition purposes will be determined only once
each year for the entire year. This status will be verified in June 2009 for the upcoming
        school year.

     8. If the name listed on the first line of this agreement is not the person financially
responsible for paying tuition, list the financially responsible person.

In order to facilitate the handling of your tuition payments, you may choose to take advantage of an
automatic draft system from your bank account.

than once during a school year a $10 administrative fee will be assessed.

If there are any questions, please call the school office at 817-451-9383.
