UNIFORM BILLING (UB-04) IMPLEMENTATION
The UB-92 billing form is being replaced. The current form will soon be obsolete.
The new UB-04 billing form incorporates the National Provider Identifier (NPI),
taxonomy, and additional codes.
PARAMOUNT GUIDELINES for the UB – 04 BILLING FORM
? When can our facility begin to use the new Paramount will begin accepting the new UB-04
UB-04 billing form to bill paper claims to billing form on June 18, 2007.
Paramount is in the process of programming our system to
