Teacher Name: Principal Name: School: I fully support the participation of the above-named teacher in GK-12 Program in Biological Sciences at the University of California, Irvine for the period of one year, starting July 1, 2008 and ending June 30, 2009. I am aware that as part of the GK-12 Program, a UC Irvine Graduate Student (GK-12 Fellow) will be partnering with the teacher to develop and implement inquiry-based lesson plans and laboratory activities in accordance with the California Science Content Standards. GK-12 Fellows will be in the teacher’s classroom for 10 hours/week during the school year. The funds received from the GK-12 Program will NOT supplant any school or district funds for our classrooms. I am aware that the above-named teacher will: Receive a stipend of up to $4,500/year for participating in all GK-12 activities Participate in an orientation session on July 10, 2008, 4:00-6:00 PM Participate in two summer institutes: July 14-18 and August 11-15, 2008 (40 hrs/week) Participate in 8 Saturday workshops: September 27, October 25, December 6, 2008, January 10, February 7, March 7, April 11, May 9, 2009 Participate in the UCI GK-12 Symposium on Monday, June 1, 2009, 5:00-7:30 PM Option to present papers about materials developed in the GK-12 Program at selected national conferences Partner with a GK-12 Fellow to work in their classroom 10 hours/week Work in partnership with GK-12 Fellow on workshop materials, presentations and lab activities Participate in ongoing project evaluation (surveys and journaling) Principal Recommendation Please comment on the above-named teacher’s classroom management and mentoring skills, ability to plan lessons in advance, work as a team, and skills in teaching science/math/technology My signature below indicates my strong commitment to support the teacher (named above) to participate in the GK-12 Program activities (described above). ___________________________________________________________ Signature of Principal _____________________ Date
