					                  NSDAR MASTER QUESTIONNAIRE FORM 2009-2010
                          Instructions for State Regents

        The attached pages constitute the new Master Questionnaire (MQ) for the year 2009-2010. This
questionnaire is used by State and National Chairmen for committee awards and official reports. It has been
modified and shortened considerably to facilitate ease in reporting. In addition, the one page per report
eliminates cutting pages apart for dissemination. Also for your convenience, at the beginning of each
committee page there is a space to report the chapter name followed by “XXX” and number of members in
the chapter followed by “ZZ” thus allowing chapters to use the “Find and Replace” feature (located in the
“Edit” menu on the toolbar) to fill in the name of the chapter and number of members without having to do it
on each form individually.
        The reporting period for chapter activities and donations is from March 1, 2009 to February
28, 2010. However, the Official Membership Count is determined on a calendar year and it is taken as of
January 1 each year. Each State Regent should have received a report of membership for each chapter in
her state from the Office of the Organizing Secretary General. Please instruct the Chapter Regents to apply
the Official Membership Count as requested in the MQ.
    State Regents are asked to either:
    1. Forward the electronic version via email to your chapters so that they may use the computer to
        answer questions and print out completed copies or
    2. Make copies of the MQ and distribute to all chapters or
    3. Notify your chapters that the MQ is located on the DAR Members’ Web Site (http://members.dar.org)
        under: Forms/Miscellaneous/Master Questionnaire.
    Some state organizations may choose to add an additional page of questions that are pertinent to their
state projects and reporting. If your state needs any additional questions, please title that page of the
questionnaire prominently (ex: CALIFORNIA STATE SOCIETY – STATE PROJECT QUESTIONS).
PLEASE DO NOT ADD QUESTIONS TO THE PAGES OF THE NATIONAL MASTER QUESTIONNAIRE
THAT FOLLOW.
    State Regents should also designate to whom chapters should return MQ reports and add that
information to a coversheet with complete instructions. Because each state’s reporting deadlines vary, it is
the responsibility of the State Regent to determine when her chapters should return the completed MQ.
Please use the National deadlines below to guide your decisions on state reporting deadlines.

TIPS FOR STATE CHAIRMEN:
  Please check with your State Treasurer to verify correct amounts for all monetary contributions or
   donations that will be reported in your state tally of the MQ and used for your state report.
TIPS FOR CHAPTERS:
  The Chapter Regent, with the aid of her officers and chairmen, is responsible for answering the MQ.
   In order to increase your chapter’s opportunity for awards, please provide detailed information for those
    committees that request summary answers in the space provided on the bottom half of the sheet. For
    longer essay type answers needing more space, the back of the sheet may be used.
   Chapters should answer EVERY committee page. If your chapter has no report for a specific committee,
    please fill in the chapter name and write “NO REPORT” across the face of that page. This will allow the
    State Regent and Chairmen to determine that all chapters have reported.
   NEW: To ensure accurate monetary reporting, all contributions and donations FROM INDIVIDUALS
    must be sent through the Development Office accompanied by the Dev.-1003 Donations and
    Contributions form. All Chapter contributions and donations continue to be sent through the proper DAR
    reporting structure: Chapter Treasurer to the State Treasurer to the Treasurer General. The exception is
    monetary assistance to Veteran hospitals/homes.
   Chapter Regents: Please check with your Chapter Treasurer in order to verify correct amounts for all
    monetary contributions/donations that will be reported in the MQ and used for your Chapter Regent’s
    report.
