					This is a document that a party can use to request to borrow a school’s materials. The
borrowing party promises to indemnify the school in case of damage or destruction of
the materials. In addition, the borrowing party agrees to hold the school harmless for
any and all claims of injury suffered by the person who may use the materials. This
document can be used by individuals or entities that want to request to borrow a
school’s materials.

       THE UNDERSIGNED, hereby requests use of the following School Materials (the
“Materials”):

2.    The undersigned requests use of the Materials, commencing on the _____ day of
__________, 201_____. The undersigned shall return the Materials to ________________
[NAME OF SCHOOL] no later than __________ a.m./p.m. on the _____ day of
______________, 201____.

3.      The undersigned acknowledges and agrees that he/she shall assume full responsibility
and liability for the Materials while in his/her possession and shall be solely responsible for any
damage to the Materials while in the undersigned’s care and control.

4.      In the event the Materials are damaged in any way or in need of repair arising out of the
undersigned use of the Materials, the undersigned agrees that he/she will be solely liable for the
costs associated with the repairs or the replacement of the Materials.

5.     The undersigned hereby agree to indemnify and hold harmless ______________ [NAME
OF SCHOOL] for any and all claims of injury suffered by any person who may use the
Materials while the Materials are in the care and control of the undersigned.

     THE UNDERSIGNED HEREBY AGREES TO THE ABOVE TERMS AND
CONDITIONS RESPECTING THE USE OF THE MATERIALS.
