                           Thrugh MFH's Healthy & Active
                           Communities Initiative, this 2-year grant
                           funds a partnership with the Clark County
                           Health Department to increase awareness
                           of nutrition and healthy behaviors, develop
                           physical activity challenges and education
Healthy & Active           programs for families, and provide

                           More than 2,000 area students are
                           learning about the effects of tobacco use
                           and being encouraged to not use tobacco

                          Access Initiative, this three-year grant
                          enables expansion of dental services to
                          uninsured, underinsured and underserved

                          supports the organization's efforts to
                          improve access to and quality of health
                          care services, promote community health
                          awareness and facilitate continuing

                          in northwest Missouri who have diabetes.

                          manage their own care. The effort is
                          funding through MFH's Chronic Care-Self-
Chronic Care              Management of Diabetes program.
                          This 3-year grant funds a diabetes self-
                          management program for about 900
                          clients in the northeast Missouri who are
                          living with this disease. This is part of
                          MFH's Chronic Care-Better Self-
Chronic Care              Management of Diabetes program.
                          This 2-year grant funds the 'Wellness
                          Recovery Action Plan (WRAP) Program,'
                          a wellness and relapse prevention effort
                          for people living in northeast Missouri who
                          are dealing with mental health issues.
                          Funding comes through MFH's Health

                          This 2-year funding enables this northeast

                           Funding supports the Project
                           Smokebusters program and provides more
                           than 6,000 students in northeast Missouri
                           with activities that illustrate the importance
                           of a tobacco-free lifestyle. This 3-year

                           With this 2-year grant, the northeast
                           Missouri organization can provide health
                           screenings to uninsured clients at the
                           Monroe County food pantry and service
                           vouchers for those needing to follow up
                           with a doctor. Funding comes through

                           Funding supports a school-based
                           substance abuse program in the Jennings
                           and Rockwood school districts in the St.
                           Louis area to help students deal with
                           substance abuse issues. The 3-year grant

                           Funding supports collaboration with the
                           Hannibal Public Schools to improve
                           mental health services offered to children
                           in the northeast region of the state. The 3-

                           Missouri organization is collaborating with
                           White Oak Counseling to address the

                           Funding enhances services currently
                           available to women in seven north central
                           Missouri counties who are victims of
                           violence, and also supports efforts to

                           American Lung Association of Central
                           States. Funding comes through MFH's
