Requesting Crash reports from the DMV
The crash reports can be requested from DMV using the request form on the website:
All motor vehicle records are subject to the Federal Driver's Privacy Protection Act
(FDPPA) and General Statute 20-43.1. A crash report, including those retrieved from the
DMV CrashWeb site are an official DMV record of the traffic accident reported in the
form and a motor vehicle record. The FDPPA and GS 20-43.1 require that personal
information in the Division of Motor Vehicles records be closed to the public. Personal
information from these records may be released to individuals or organizations that
qualify under one of the fourteen exceptions as follows. Failure to comply with the
provisions of the FDPPA could result in the loss of access to the DMV CrashWeb site.

Request for Motor Vehicle Information
Effective September 13, 1997, all motor vehicle records are subject to the Federal
Driver’s Privacy Protection Act (FDPPA) and General Statute 20-43.1. A crash report is
a motor vehicle record. The FDPPA and GS 20-43.1 require that personal information in
the Division of Motor Vehicles records be closed to the public. Personal information
from these records may be released to individuals or organizations that qualify under one
of the fourteen exceptions listed on the back of this form. These exceptions are
summarized statements of permissible uses.

FEDERAL DRIVER'S PRIVACY PROTECTION ACT
Enacted by Congress August 24, 1994
Chapter 123, Section 2721 requires that personal information DMV's records be closed to the public. This
refers to photos, social security numbers, drivers license numbers, names, addresses, telephone numbers
and medical information.
General Purpose: Division of motor vehicle, and any officer, employee or contractor, therefore, shall not
knowingly disclose or otherwise make available to any person or entity personal information about any
individual obtained by the division in connection with a motor vehicle record.
Permissible Uses: Personal information SHALL be disclosed for use in connection with matters of:
A. Motor vehicle or driver safety and theft
B. Motor vehicle emissions
C. Motor vehicle product alterations, recalls or advisories
D. Performance monitoring of motor vehicles and dealers by motor vehicle manufacturers
E. Removal of non-owner records from the original owners if motor vehicle manufacturers to carry out
purpose of the Automobile Information Act, the Motor Vehicle Information and Cost Saving Act, the
National Traffic and Motor Safety Act of 1966, the Anti-Car Theft Act of 1992 and the Clean Air Act.
Exceptions: Personal information MAY be released for the following reasons
1. For use by any government agency, or any private person or entity acting on behalf of a Federal, State,
or local agency in carrying out its functions.
2. For use in matters of motor vehicle or driver safety and theft, motor vehicle emissions, motor vehicle
product alterations, recalls or advisories, performance monitoring of motor vehicles, motor vehicle
parts and dealers, motor vehicle market research activities, including survey research, and removal of
non-owner records from the original owner records of motor vehicle manufacturers
3. For use in the normal course of business by a legitimate business, but only:
a. To verify accuracy of personal information
b. To obtain correct information, but only for purposes of:
1. Preventing fraud by the individual
2. Pursuing legal remedies against the individual
3. Recovering on a debt or security interest against the individual
4. For use in connection with any civil, criminal, administrative, or arbitral proceeding in any Federal, State
or local court or agency (includes the execution or enforcement of judgments and orders or court orders)
5. For use in research activities and statistical reports
a. Personal information must not be:

6. For use by insurance companies in connection with claims investigation, antifraud activities, rating or
7. For use in providing notice to owners of towed or impounded vehicles
8. For use by private investigators or licensed security service
9. For use by employer to verify information regarding CDL
10. For use in connection with private toll facilities
11. For any other use if person has opportunity to refuse disclosure on DMV forms (Prohibited by NC
General Statute 20-43.1).
12. For bulk surveys, marketing or solicitations, if person has opportunity to refuse disclosure on DMV
forms. This disclosure is for titles and applications for leased vehicles issued on and after July 1, 1998.

13. For use by any requester that has obtained written consent of the individual to whom the information

Retrieving Crash reports from the DMV CrashWeb Site
Authorized users can retrieve Crash Report images from the DMV Crashweb web site
To View a Single Crash Report
From the Crashweb main screen click the “External User Crash Report Requests” button.

Enter your User ID and Password. A User ID and Password is provided by DMV Traffic
Records Section. Contact Operations Support at (919-861-3084), to be given
authorized access to the CrashWeb before logging in for the first time. Enter the User ID
and Password and click the “Submit” button.
The screen shown above appears. This page explains what CrashWeb will enable a user
to do, and instructions on how to view Single Crash Reports, how to create batches of
crash reports and how to maintain those batches. Click the “Single Crash link” on the
left-hand navigation pane.

The screen shown above appears. Enter the Crash ID for the crash report that is to be
viewed in the space next to "Enter Crash ID:" Press the “Submit button.” An example is
After pressing the "Submit" button, the screen is updated to show what documents are
available for the Crash ID that was provided. In the example above, Crash ID: 99002833
has an Initial Filing document available for viewing. This is indicated by the View link
next to the Initial Filing identifier beneath the Documents available for crash ID
99002833 heading. Click the "View" link.

The File Download dialog box appears as shown above. Click the "Open" button to view
the image using the Internet Browser's default image viewer. Click the "Save" button to
save the image to a specific location to view at a later time.
*Note The default image viewer is WangImg.exe. If this has been changed, the
image may not be able to be viewed unless it is saved first.
If the Open button was selected, the image will appear in it's own window as shown
above. If the image was saved, browse to the location containing the saved image and
double-click on the image to view it.

To Create a Batch of Crash Reports
Several crash reports can be combined into one image file. To create a batch of crashes,
click the "Create New Batch" link on the left-hand navigation pane shown on the screen

Create a name for the batch of crashes by entering a name in the box next to "Batch
Name". Input the crash ID's for the batch in the text box next to Input Crash ID's. The
batch can contain supplementals and can be sorted by Crash ID by clicking in the
appropriate box to select the options listed.
In the example shown above, the Batch Name is 20040507. The batch is composed of the
following crash ID's: 99003844, 99003746, 99003712, 99002374, 99001273. The batch
is going to be sorted by Crash ID as indicated by the check box next to "Sort by Crash
ID". Click the "Submit" button.

After the "Submit" button has been pressed, the screen is updated as shown above. The
batch number, batch name, date submitted, date processed and current status is displayed.
In order to view the batch, the user must click on the "View Existing Batches" link on the
left-hand navigation pane.

The screen shown above appears. The table shows the Batch number, the Batch Name,
the Date Submitted, the Status and a Delete check box option. The 20040507 batch is
processed and is ready to be viewed. In order to view the image containing all of the
crash ID images that were included in the batch, click the batch No. link in the table :

The batch details screen is shown above. In order to view the image containing all of the
crash ID images, click the "View" link next to 600012189_1.tif beneath the "Merged
Documents for viewing and printing" title.
The File Download option screen appears. Click the "Open" button to open the Image
file to view the image containing all of the crash ID images, or click the "Save" button to
save the image file to a specific location for later viewing.

The batch image is displayed as shown above. Using the image viewer's Next Page
button, scroll through the image to view all of the images for each crash ID that was
included in the batch. Close the batch image to return to the batch details screen.

To Delete an Existing Batch
After logging into Crashweb , the main screen appears. Click the "View Existing
Batches" link on the left-hand navigation pane to view all batches that have been created
specific to the user that has logged in.

Batch 600012189 appears in the "Status of Current Requests" table as shown. To delete
this batch, click in the area beneath "Delete" to check the box.
Batch 600012189 is marked for deletion as shown. If more than one batch exists, several
batches can be marked for deletion at the same time. In the example above only one
batch, 600012189 is marked for deletion. To delete this batch, click the "Delete Selected
Batches" button within the "Status of Current Requests" table.

After the "Delete Selected Batches" button has been clicked, the screen updates to show
that the batch has been deleted. The "Status of Current Requests" table will no longer
have an entry for batch 600012189.
