For those receiving an honorarium, a current W-9 form must be on file in Accounts Payable. This form has current address and taxpayer information. In addition to the W-9, a copy of the agreement/contract should be included. Contracted services (7010) Before any payment is made to an independent contractor, a current W-9 form must be on file in Accounts Payable. Some examples include caterers, musicians, construction companies, painters – any one who provides a service. In addition to the W-9, a copy of the agreement/contract should be included. Approval: Make sure you have budget privileges to approve the form. Please approve with a signature, not initials. For reimbursements to self or travel advance requests, direct supervisor’s signature is required. If unavailable, supervisor’s supervisor can approve.
