Overview:
The Center for Risk Reduction (CFRR) utilizes an innovative approach that responds rapidly and
strategically to reducing high risk behaviors for vulnerable populations in our community. We
recognize that there are many steps along the way to good community health. We believe that health
comes from a combination of disease prevention, health education, and quality care. To that end, CFRR
responds to the unmet needs in the community for direct services, prevention education and support. In
particular, we focus on people who are at high-risk for substance abuse and infectious diseases and child
abuse/neglect. The center has several full-time openings for Outreach Specialist/Case Managers as part
of ProjectConnect. Community outreach is conducted to disseminate information and engage minority
women (Hispanic and African-American) who are not in treatment, who use or abuse substances, and
who have or are at-risk for HIV/AIDS. Services are designed to utilize strategies for enhancement of
personal risk reduction efforts and to support and maintain behavior change. The Center houses several
programs: Orgullo, HIV/AIDS Case Management, HIV/AIDS Prevention Education, Hartford Youth
Project, Youth on Health, Dutch Point Program, Parent Aide Program, Mayor’s Healthy Communities
Initiative and Farm Workers Program.
The Organization:
Established in 1978, the Hispanic Health Council (HHC) is a community-based, non-profit organization
located in Hartford, Connecticut. HHC's unique organizational model integrates policy advocacy,
community-based research and evidenced-based service to address health inequities experienced by
diverse communities. To carry out this charge, the organization has established the following Centers of
Excellence: Center for Community Health Research; Center for Women & Children’s Health; Center for
Community Nutrition; Center for Risk Reduction and the Latino Policy Institute. The Hispanic Health
Council is also a founding partner with the University of Connecticut in the Center to Eliminate Health
Disparities Among Latinos (CEHDL)
Duties and Responsibilities:
• Outreach, engagement, dissemination of prevention information, condom dispersal,
referral and on-site HIV/AIDS rapid testing
• Works closely with small business owners and the local police force to form
collaborative relationships that produce information on where to find women at risk of
HIV/AIDS
• Orients women to the goals of the project and screens for eligibility
• Supports the development of client goals and has a plan for achieving them
• Collects and documents information regarding outreach, pretreatment, case management,
support services and referrals
• Participates in all ProjectConnect treatment team and case review meetings
• Completes and maintains accurate records of referrals in a timely manner
• Maintains confidentiality of all client information as per federal and state laws
• Follows all established HHC policies and procedures
• Carries out additional duties as identified by supervisor
Qualifications:
• BA in human service field preferred, but not required
• Knowledge of Hartford places where at-risk Hispanic and African-American women can
be found
• Knowledge of HIV/AIDS, substance abuse issues and case management systems
• Ability to relate to people of different cultural and ethnic backgrounds
• Ability to use computer for data entry and reporting preferred
• Own transportation and valid CT driver’s license
• Bi-lingual: Spanish/English
Experience:
• Three years of work experience in HIV/AIDS or substance abuse case management
and/or street outreach
• Bilingual and bicultural/African-American candidates are encouraged to apply
• Familiarity with local regional health and social service resources
