Food Vendors Certification:
I certify that the food items I offer for sale in the Charleston Farmers Market, as per the attached
list, will have been prepared by me in a South Carolina Department of Health and Environmental
Control (DHEC) or Department of Agriculture approved kitchen.

I have read, understand, and agree to comply with the 2010 Charleston Farmers Market
General Rules and Regulations as well as the Food Vendor Rules and Regulations. I
understand the violations and sanctions, including suspension and disqualification. I understand
selling privileges can be revoked by the City of Charleston at any time.

I have attached a copy of my City of Charleston business license, a copy of the 2010 DHEC or
Department of Agriculture certification, and a list of the items I plan to sell during the 2010
Season of the CFM. I understand that my kitchen is also subject to inspection by the CFM staff
and DHEC.

I understand that no food vendor is guaranteed acceptance in the CFM and is subject to
approval of the CFM Management. Food Vendors that are accepted in the 2010 CFM will be
notified by the Office of Cultural Affairs. I understand that all Food Vendors accepted into
the 2010 CFM will be expected to participate in all scheduled market dates.
The Charleston Farmers Market does not accept applications from food vendors residing
outside the Lowcountry counties east of I-95. These counties include Charleston,
Dorchester, Berkely and Colleton Counties.

Please enclose the following materials to be considered for participation in the 2010 Charleston
Farmers Market:

       A copy of the vendor’s 2010 DHEC or Department of Agriculture certification

       A list of the items to sell at CFM

       Information for CFM website

       If accepted, a copy of vendor’s City of Charleston business license will be required
before the beginning of the market.

Proof of vendor’s insurance or signed insurance waiver
