We have been notified by XYZ Insurance Brokers about a possible claim under your Householders
insurance policy which is insured with QBE Insurance Limited.

As a further service to you, we have been appointed by XYZ Insurance Brokers to assist in the
submission of your claim. It will be our responsibility to:

   Ensure the smooth and efficient handling of your claim by the Insurance Company.
   Ensure that you are aware of any and all documentation required.
   Provide you with regular updates on the progress of your claim.
   Respond immediately to any queries or possible delays in the handling of your claim.

We have advised QBE Insurance Limited of a possible claim and advise that you:

       Must take all reasonable precautions to prevent further loss, damage or liability
       Must not authorise repairs, other than emergency repairs necessary to prevent further loss
       Must not arrange for replacement of any insured property
       Must not admit liability if an accident has occurred

Attached is a Claim Form for your attention. Please complete this form in full and return it to this
office together with the following documentation in support of your claim;


       Copy of valuation or purchase invoice for the lost Albert Chain

       Copy of Police notification form
