  on self-management interventions and
  what agencies want out of it
Self-management Mapping
 Part of a range of DHS initiatives including
early intervention projects, HARP CDM, Health
 Promotion/prevention
 Two PCPs have undertaken mapping pilot
 projects: outer east and the south east
 All PCPs, GP Divisions are funded for ICDM –
 mapping is the first step
 Challenge is to coordinate the various existing
 and new approaches to improve people’s
 experience of the system and their management
 of their health
DHS Aims:
 Improve understanding of self-
 management
 Gather a state-wide picture of service
 system capacity
 Better understand training and
 workforce issues
 Identify referral pathways
 Identify service gaps
DHS Role
 Develop approach, definitions,
 templates and guides
 Consolidate the information and
 develop a data base
 Support PCPs, analyse data and
 report back

 Inform members and others
 Determine what additional
 information will be collected with the
 SMM survey and why
 Develop a project plan, identify
 contacts and assist agencies
 Forward information to DHS
 Plan next steps for ICDM
