Request for routing the student visa application through the School of Business and Management, Hong Kong University of Science and Technology
I, _______________________________________(name), have been selected by ___________________________________________(name of your home university) to participate in the MBA exchange program with the School of Business and Management, Hong Kong University of Science and Technology (SBM-HKUST) for 2007 Fall. I would like to route my student visa application through SBM-HKUST. By doing this, I understand that SBM-HKUST has to pick up the student visa label from the Hong Kong Immigration Department when my application is approved. SBM-HKUST will then send the visa label to me by registered mail. I will inform SBM-HKUST if my mailing address is changed at a later time. It is my responsibility to pay the visa fee of HK$160. I agree to pay this amount to SBMHKUST by credit card so that they can have the money to pick up my visa label when it is ready. Should the visa fee be revised at a later date, I agree to pay the balance to SBM-HKUST upon my arrival at HKUST. I understand that the visa money is non-refundable when it is paid to the Hong Kong Immigration Department even if I have decided to withdraw from the exchange program at a later date.
