					              PROCEDURE FOR NHW COMMUNITY
            CRIME & SAFETY ALERT REPORT FORMS
To enable the community to work more closely with the City of Cockburn Neighbourhood
Watch network, we are providing you with these Community Safety Alert Report Forms.

The form will facilitate a more efficient and timely way of reporting incidents of crime to the
Police and Security who can then be more efficient in the fight to Clean Up Crime in your
Suburb.
   • TO TAKE ADVANTAGE OF THIS PROGRAM ALL YOU NEED TO DO IS:-
           1. Complete the CRIME & SAFETY ALERT FORM to report any incident that
              threatens the safety of any individual or the community within your
              immediate neighbourhood.
           2. The form can only be considered as completed when the person who is
               reporting the incident has included their CONTACT DETAILS in the space
               provided at the bottom of the form. (see Confidentiality and Police Contact
               details at bottom of the form).
           3. Send your COMPLETED SAFETY ALERT FORM to your LOCAL
              NEIGHBOURHOOD WATCH REPRESENTATIVE or direct to the CITY OF
              COCKBURN COMMUNITY SAFETY OFFICER. (see contacts list)

   • HOW IT WORKS:
            1. The details of your report will be provided to the City of Cockburn’s
               Community Safety Manager, who will generate a POLICE REPORT of the
               incident that can then be distributed to your immediate neighbours. (your
               details will not be included)
            2. The Police will be asked to approve the INCIDENT REPORT for
               community release after which the Incident Report will be included on the
               City of Cockburn Neighbourhood Watch COMMUNITY SAFETY FLYER.
               (see attached sample).
           3. The Flyer will be provided to your local Neighbourhood Watch
              Representative who will distribute the Flyer to you and your neighbours
              letterboxes. This will enable you and your neighbours to be aware of any
              safety concerns in your area that have been brought to the attention of the
              Police and the Cockburn Security Service.
            4. You do not need to be a member of Neighbourhood Watch to complete
                one of these forms.
