There are two ways: Print one from the PHHS Patriot Exhibition website (http://www.patriotexhibition.com) Pick one up at a local place of employment
What Items Will I Need to Complete the Application?
Social Security Number (you may make one up)
Schools Attended
You should have a SSN however. Ask your parents if you have one.
Name and address of each
Dates of attendance
Special courses taken
If you don’t have a SSN get one call 800-772-1213. You cannot get a SSN on-line

What is the First Step to Filling Out a Job Application?
Make a photocopy of application – you will make mistakes! Use blue or black ink and write LEGIBLY. Print information. Read each question or box carefully.
What Should I Avoid Doing?
No blank spaces Don’t lie! May result in immediate termination. Don’t misspell words or use poor grammar – PROOFREAD!
What if I’m Not Really Applying for a Job?
Purpose of activity: learn how to properly complete a job application. If creating a fictitious job application – be realistic! Apply for a position suited for a teenager.
