					Tobacco Prevention & Control
Strategic Plan Evaluation Plan 2005-2008
This evaluation plan was created from priorities identified during a local six-month strategic
planning process. Statewide short-term outcome measures were standardized across
evaluation plans so results could be evaluated and measured based on indicators and
activities being conducted. Due to small samples/large confidence intervals plans were not
required to have target numbers. When available local measures were used to measure
priorities.

This evaluation plan can be used as a guide in structuring and selecting local activities. In
future years there will be opportunities to revise the evaluation plan if priorities change or if
data indicates a need to assess the program activities. This evaluation plan will not be used
by The Department of Health to designate funding or determine overall success or failure of a
local tobacco prevention program. The Department of Health Tobacco Prevention & Control
evaluation team will produce annual update reports based on the current priorities in your
