If several people share in the expenses list each person’s contribution. List name and address for each person requiring a tax receipt. (Use separate sheet if necessary) Tax receipts will be issued after gift card validation is confirmed by the Calgary Women’s Emergency Shelter. While all gifts are appreciated, gifts from children cannot be tax receipted.
*Unfortunately we are unable to issue tax receipts without actual store receipts – this includes receipts for gift cards and food vouchers/grocery gift cards. It is mandatory that this form and the above accompany your 2008 Adopt-a-Family donation.
