   · Obtain account information on my accounts via the Member Contact Centre
   · Authorise transactions on my accounts via the Member Contact Centre
   · Access the Account Information Line to perform transactions and/or obtain information on my accounts
 I agree that all accounts under this membership are to be linked to this service.
 Passcodes must be kept confidential and not disclosed to any person other than appropriate employees of Community CPS Australia Ltd (Community
 CPS and incorporating United Community).
 I acknowledge and agree that my signature on this form signifies receipt and acceptance of the appropriate product information, terms, conditions, fees
 and charges documents which form part of this application.
 Choose your own temporary default passcode by writing down 4 to 8 numbers only in the boxes at the bottom of the page.
 When you access the Telephone Banking system for the first time you may be asked to change your temporary default passcode. If you are not prompted
 to change this temporary default passcode, we recommend that you select option 5 to change your passcode. We also recommend that you choose a
 passcode that would be difficult for anyone else to guess. Please do not choose numbers from your member number, birth date, or any simple sequence
 as these may not work with the Telephone Banking system.
 Your passcode can be changed at any time by calling the Account Information Line on 13 14 02 and selecting Option 5.
