Second Skin Dynamic Splinting and its Application for Children and
                   Adults with Hemiplegia

Splinting has played an extensive role historically in our therapeutic approach with clients with
hemiplegia. Second Skin Dynamic upper limb and lower limb splinting has become very popular as a
choice for clients around the UK who are working to maximise functional improvements.

Second Skin Dynamic splinting for children and adults with hemiplegia centres on developing the
following areas:

    •   Improved upper and lower limb posture.
    •   Improved sensory awareness in the upper and lower limb.
    •   Improved patterning of movement when moving away from the centre of gravity.
    •   Improved bilateral arm use and bimanual hand use in specific skill areas e.g. play, leisure, self-care
        skills, work related skills.
    •   Improved bilateral weight bearing skills.
    •   Improved grasp/sustained hold and active release skills.
    •   Improved step through and mid gait phase in walking.


For many clients we are working to support their posture and reduce the impact of their altered
muscle tone. Splinting is frequently a long-term approach for our clients in order to develop their
functional skills or to prevent deformity. The challenge for our therapists has been to individually
design dynamic splinting with maximum contour and comfort to enable consistent splint wear.

Second Skin splints are individually designed for clients according to their postural and tonal needs.
Careful consideration is made to individual targeted goals.

Second Skin splinting is appropriate for clients with all aspects of muscle tone i.e. spasticity, low
muscle tone, dystonia, ataxic or involuntary movement.

                                             SUSAN
Susan presents as a young adult with issues of upper limb spasticity. She is able to initiate active
finger clearance and a weak grasp using combinations of wrist flexion and ulnar deviation.

Our key focus for Susan was to stabilise her wrist into greater midline enabling improved alignment of
her finger flexors and extensor muscles. We also looked to de-rotate her thumb enabling Susan to
develop her controlled grasp strength. These skills were critical for Susan’s secretarial work tasks
and her independent living skills.

                                             ALICE
Alice struggles to maintain her right foot posture when walking. She has a tendency to circumduct at
her hip to compensate for her difficulty in clearing her toes. Consequently Alice takes her weight
primarily through her toes and struggles to heel strike.

Alice’s individually designed Second Skin dynamic foot splint has been extremely beneficial in
improving her heel strike and symmetrical weight bearing. Her dynamic balance and sensory
awareness has improved and our focus is now on improving her high level mobility skills, walking on
uneven surfaces etc. We are also keen to continue to work on her walking endurance.

                                                BROOK
Brook is a young boy who presents with dystonia in his right arm and hand. He works hard to use his
arm and hand in bimanual tasks but struggles considerably with his lack of motor control.

Brook’s arm and hand splints have been individually designed to set boundaries in his movement. It
is critical for the splints to allow Brook to actively move through all ranges of upper limb movement
enabling him to experience and develop his gross upper limb skills. He continues to respond very
well to his splinting.
