Exit Interview for Separating or Transferring Employees PURPOSE The purpose of conducting this survey is to help BAS Unit Heads and the Office of Planning and Analysis (OPA) do the following: • assess the workplace climate in the BAS division; • identify ways of improving our organization’s climate and management; BAS Exit Interview • understand the factors that influence or contribute to BAS employees’ decisions to leave the division or the campus; • identify ways of improving conditions of employment within our division. Your participation and feedback is voluntary. It is also extremely valuable, and important in helping us to achieve these goals. This survey is designed to provide feedback only, and is not a forum for negotiating or re-negotiating any terms surrounding your separation from the University. INSTRUCTIONS • Complete the on-line version of the survey by going to the OPA website (http://bas.ucsc.edu/ opa) and clicking on the link to the exit interview; or, • Complete a paper version. Paper versions are available in both english and spanish from Staff Human Resources Service Team A. After completing the survey, please send it to the following address: BAS Office of Planning and Analysis University of California Santa Cruz, CA 95064 To help ensure anonymity, data from completed paper versions will be entered into a web-based database, and the originals will then be destroyed. Data from the exit interviews will be compiled annually, and amalgamated responses from each unit will be provided to the appropriate Unit Head. Despite our efforts to make sure your identity remains anonymous, please be aware that we cannot guarantee anonymity if the turnover rate is low and/or the department from which you left is small.
