Notes: 1. For bisessional schools, please specify morning or afternoon session.
2. Please delete as appropriate.
3. Please put a “√” in the appropriate box. If a venue outside the school is used for
training, please specify the name of the venue and arrange transport to and from
the venue for students.
4. Please fill in all the training dates (except school holidays) according to the set-up
of the course. 【
A one-week interval should be allowed in-between. For example:
16/9, 23/9, 30/9, 7/10】
5. Please fill in the time according to the duration of each lesson. The time can be
fixed in the following periods: from 8 am to 5 pm from Monday to Friday and
from 8 am to 12 noon on Saturday.【For example: 8:30 am – 10:30 am】
Remarks: 1. A separate form is required for each sport. In case of oversubscription, enrolment shall be
decided by ballot.
2. The duly completed enrolment form for each course / activity, together with a cheque
(payable to the respective National Sports Association 【Please refer to Appendix I for
details】 and with the school name marked clearly on the back) for full payment of the course
/ activity fee, should be sent by post to the School Sports Programme Unit, 1/F, Leisure and
Cultural Services Headquarters, 1-3 Pai Tau Street, Sha Tin.
3. Schools should submit their enrolment forms before the respective deadlines, otherwise their
applications may not be processed on time. For late applications, the LCSD will only make
arrangements as far as practicable.
4. If schools have problems in finding suitable venues, they may consider applying for free use
of the arena, activity rooms, squash courts, etc at the nearby sports centre for the period from
7 am to 5 pm from Monday to Friday (except public holidays, July and August) under the
Free-Use Scheme introduced by the LCSD.
