SANTA MONICA/WESTSIDE GUIDE TO HEALTH CARE AND SOCIAL SERVICE AGENCIES Community and Cultural Services Department Human Services Division April 2004 The City of Santa Monica is pleased to present to you this Resource Guide, an annually updated listing of services in Santa Monica and on the Westside. This guide is intended as a starting point and we encourage you to contact each agency for more information about its current services and programs. Consider the following questions: 1. 2. 3. 4. 5. 6. 7. 8. Does the agency accept all types of insurance, Medi-Cal or participating in an HMO? What current language capabilities do they have? (i.e., Spanish, Korean) Do they have a sliding fee scale? Is there a waiting list? Do they provide transportation? What are current eligibility requirements? Are there any restrictions? What are the hours of operation? What other services are offered at the agency? Also, when looking for client services, search under different categories. For example, a client with Alzheimer’s Disease may find services under both Older Adult Services and Disability Services. Clients who are veterans may also be entitled to special programs administered by the Veteran’s Administration. This is not a complete list of resources in Santa Monica and on the Westside.
