					                                      JOB DESCRIPTION

TITLE: Community Transition Specialist

REPORTS TO: Manager of Community Transition Services

POSITIONS SUPERVISED: None

GENERAL FUNCTIONS: Provide comprehensive services to students in community transition
program, including classroom activities, job development, job retention, and community based
assessment.

SPECIFIC FUNCTIONS:

      Provide appropriate classroom activity using assigned curriculum

      Conduct community-based assessments for participating students

      Provide job development services as needed

      Provide job retention services as needed

      Attend IEP meetings as required

      Provide job and social skill training to all students

      Secure and provide job shadowing experience for students

      Provide ADL training to students, i.e. cooking, cleaning, computer skills, etc.

      Plan and implement social outings for students

      Transport students as needed to vocational sites and social outings

      Complete documentation in timely manner

      Maintain appropriate CARF, DRS, DMH, DDD, DPA, MRO and school standards

      Perform additional duties as assigned by the Manager of Community Transition Services


QUALIFICATIONS: BA degree in human services/education field preferred. High School
diploma or GED & prior experience in community-based services will be considered.
Experience with students aged 18-21 preferred. Driving record and auto insurance in accordance
with agency policy. A criminal background check may be run, if applicable. EOE.

SHIFT HOURS: 7:00am-3:30pm, Monday-Friday, other hours as required

STATUS: Non-exempt
