1. Purpose
To document all identified and reasonably foreseeable OHS hazards and risks at UNSW. The
OHS Hazard and Risk Register will be a valuable tool in assisting UNSW to plan and manage
these hazards. It will assist local units to implement requirements of the OHS Risk
Management Program.

2. Scope
The UNSW OHS Hazard & Risk Register Procedure applies to all UNSW staff.

This Procedure applies to all identified or foreseeable hazards and risks that can arise from:
• any UNSW work environment;
• any UNSW work, research or teaching activity;

3. Definitions

Work Area – any identified room (or group of rooms) at UNSW were activities are undertaken
or hazards are identified eg office, laboratory, reception area, store room, animal house.

4. Procedure

         4.1      Updating or Creating an OHS Hazard and Risk Register

         There are 5 steps in creating the UNSW OHS Hazard & Risk Register.

               1. Hazard identification, assessment and control (See UNSW OHS Risk
                  Management Program);
               2. Creating or updating the Workplace OHS Hazard and Risk Register using the
                  information identified in the risk assessment;

               3. Creating or updating the School or Unit OHS Hazard and Risk Register using
                  the work area OHS Hazard and Risk Register data;
               4. Creating or updating the Faculty or Division OHS Hazard and Risk Register
                  using the School or Unit OHS Hazard and Risk Register data;
               5. Updating the UNSW OHS Hazard and Risk Register with Faculty and Division
                  OHS Hazard and Risk Register data.


         4.2      Step 1 Hazard Identification, Assessment and Control

         All hazards that are identified by any activities outlined in the UNSW OHS Risk
         Management Program must undergo a risk assessment as outlined in the UNSW OHS
         Risk Management Program - OHS Risk Assessment and Control Procedure.

         4.3      Step 2 Creating or updating the workplace OHS Hazard and Risk Register

         All hazards that are identified by any activities outlined in the UNSW OHS Risk
         Management Program must be registered on the Workplace OHS Hazard and Risk
         Register.

         The work area OHS Hazard and Risk Register is populated with the information
         obtained from the risk assessment that has been undertaken after hazard identification.


         4.4      Step 3 Creating or updating the School or Divisional Unit OHS Hazard and
                  Risk Register

         Each School or Divisional Unit is required to maintain a School or Divisional Unit
         OHS Hazard and Risk Register that will be updated at least annually.

         The School or Divisional Unit OHS Hazard and Risk Register is populated with the
         data from all work area OHS Hazard and Risk Registers.

         4.5      Step 4 Updating the Faculty or Division OHS Hazard and Risk Register with
                  School and Unit OHS Hazard and Risk Register data.

         Each Dean and Division Manager will coordinate the collection of all School or Unit
         OHS Hazard and Risk Registers annually to develop the Faculty or Division OHS
         Hazard and Risk Register. The Faculty OHS Hazard and Risk Register will be a
         summary of the School and Divisional Unit OHS hazard and risk registers.


         4.6      Step 4 Updating UNSW OHS Hazard and Risk Register with Faculty and
                  Division OHS Hazard and Risk Register data.

         The OHS Manager will coordinate the collection of all Faculty and Division OHS
         Hazard and Risk Registers annually to develop the UNSW OHS Hazard and Risk
         Register.

         The UNSW OHS Hazard and Risk Register will be a summary of the Faculty and
         Division hazard and risk categories.

         5. Responsibility

         5.1      Deans and Divisional Managers
                  Have the responsibility to ensure that:
                  • an OHS Hazard & Risk Register is developed and maintained for each
                     School or Unit within their Faculty or Divisional;
                  • coordinating the provision of all School or Unit OHS Hazard and Risk
                     Registers to the OHS Manager for addition to the UNSW OHS Hazard and
                     Risk Register as part of the annual reporting process.

         5.2      Heads of Schools, Division Managers
                  Have the responsibility to ensure that:
                  • they maintain a School or Unit OHS Hazard and Risk Register;
                  • hazards in all work areas (eg laboratories, workshops, office areas) are
                     included on the School or Unit OHS Hazard and Risk Register;

         5.3      Workplace Manager/Supervisor
                  Have the responsibility to ensure that:
                  • all identified hazards or reasonably foreseeable hazards in their work area
                     are risk assessed using OHS Risk Assessment and Control Procedure.
                  • the work area OHS Hazard and Risk Register is updated with the identified
                     hazard and risk assessment information.

         5.4      OHS & WC Manager
                  Has the responsibility to ensure that:
                  • The UNSW OHS Hazard and Risk Register is updated annually with all
                     Faculty and Divisional identified hazards and risks;
