Develop Common Formative Assessments and agree upon your timeline for the assessments – place copies in your PLC
notebook.

Important Items to Remember:

 If a student and teacher does NOT hear or do anything with the feedback on a formative assessment, it is NOT a formative
      assessment.

 What will you do for the students who do NOT reach the set level of proficiency for the Essential Learning?

 What will you do for those students who HAVE already reached the set level of proficiency on your Common
           Assessment(s)?

 Refer to the 20 examples of Common Formative Assessments that Rob and Teresa shared to support your work (refer to
“Assessment „for‟ Learning Formative Assessment Ideas” found on docushare or your PLC notebook)

 Consider working within your PLC to create a Common Unit Planning Guide (refer to the form found on docushare or your
      PLC notebook)

Looking at your PLC students’ results from the Common Assessment(s):

 Describe the students’ level of proficiency on the Common Assessment(s).

 Describe the students’ level of proficiency on the Common Assessment(s) for the following subgroups:
       (M/F; Ethnicity; Sped/Regular Ed)

What will you do for the students who did NOT reach the set level of proficiency for the Essential Learning?

What will you do for those students who HAVE already reached the set level of proficiency on your Common
Assessment(s)?

Process as a group. What do you want to keep/remember for the next time that you teach this Essential Learning?

* Review student data to select the next Essential Learning for the next SMART goal.
