Follow one of the methods for delivering your complaint as explained below.
Please do not send any documentation; send only the complaint form. If
additional information is required by the financial institution to resolve your
complaint, they will contact you directly.

Submit Electronically
www.dfi.ca.gov/directory/
Fill out the complaint form, save the form to your hard drive.
Open your e-mail account and e-mail the form to:
Consumer.complaint@dfi.ca.gov
Insert/attach your file/complaint and send.
You will receive an e-mail receipt confirmation.

Submit Via Mail or Fax
Open the link on www.dfi.ca.gov/directory/
Fill out the complaint form.
