                                   Renovation/Repair Work Form
In the interests of assuring both structural integrity and resident safety and in accordance with house rules, 1705
Lanier Place, NW requires owners to notify the Board of Directors of changes they plan to make to their apartments,
particularly those that may affect the entire building or the neighbors of the apartment in which work will be done.

As further stated in the house rules, the work you do must be in accordance with D.C.’s current housing codes. You
are advised to use a licensed and bonded contractor for any planned work that would involve structural, electricity,
or plumbing changes. You are also responsible for working with your contractor to obtain any required permits to
perform electrical, plumbing, or structural work.

You can be held liable for any work that you have done that does not meet D.C.’s current housing codes and/or that
impinges on the safety of your neighbors. The best way to ensure that the work is done safely and according to code
is to use a licensed and bonded contractor.

Please notify your neighbors of the work you plan to do, how long you expect it to last, and the hours during which
they can expect the most noise and disturbance.

Examples of the kind of work you should notify the Board about include (but are not limited to):

   Electrical work: Rewiring your unit, upgrading from a fuse box to a circuit breaker box, installing new electrical
    fixtures (lighting, ceiling fan, garbage disposal, etc.)

   Plumbing work: Replacing pipes, especially if you are replacing metal pipes with PVC pipes; moving plumbing
    fixtures, such as toilets, sinks, bathtubs, dishwashers, etc.

   Structural changes: Removing walls, closets, etc.

Fill out the application on the reverse side and return it to the President of the Board of Directors one week prior to
the monthly Board of Director’s meeting. Board meetings are held on the third Monday of every month. The Board
will review your application at the next meeting after you give it to the President. You may be asked to attend the
meeting to discuss the proposed work.

Finally, keep in mind that you must arrange for disposal of any debris from your renovations (e.g., old plumbing
fixtures, pipes, drywall); it may not be placed in the trash room. Call Delbe for the name and number of a trash
hauling company to arrange for a pick up. House rules give the Board of Directors the right arrange for bulk trash
pick up and bill you for the charges if you do not take care of this yourself. Also, you are responsible for
supervising the contractors you have hired while they are on the premises. In particular, d you must ensure they
understand that propping open the front door is strictly forbidden unless they have someone to watch the entrance.

I am aware of the house rules regarding performing renovations, that work must be performed to comply
with D.C. codes, and that I am liable for any work I perform or arrange to have performed should it cause
hazardous conditions in my unit or to other units in the building. I also understand I am responsible for
any damages caused by my contractor, his or her sub-contractors, employees and/or any other company
or person involved in the renovation of my apartment.
