                                         Trade Adjustment Assistance (TAA)
                             CLASSROOM TRAINING RESPONSIBILITIES
     You have been approved for TAA funded training in the                                              program
     at                                                        . TAA can authorize one training program only and
     you cannot change your approved training or funding will be terminated.

     Should it become necessary to request a Temporary Leave of Absence due to your or a family member’s illness
     you will not be eligible for Trade Readjustment Allowances (TRA) during this period.

     A participant who withdraws from training to return to work for a new employer or for their trade affected
     employer may be allowed to return to TAA funded training. Each request will be viewed on a case-by-case
     basis. If you return to permanent full time work which pays 80% of your prior wage and requires skill levels
     equal to or higher than your Trade affected employment you are no longer eligible to receive TAA services. The
     purpose of TAA training is to prepare you for your next job. Upon completion of this training, you are expected to
     seek employment. It is imperative that you report when you return to work to your TAA Service Provider.

     As a participant in TAA funded training you are bound by the responsibilities listed below. Please initial each
     box. Keep in mind your TAA Service Provider is available to assist you if you have any questions or concerns
     regarding the following:

          Notify your TAA Service Provider of any difficulties you encounter in your training.

          Provide copies of each class’s syllabus, registration and grades for each quarter/semester to the TAA
          Service Provider.

          Submit weekly or monthly attendance reports as directed by your TAA Service Provider

          Attend classes during the summer if classes are available and prescribed for your training program and
          indicated on your training contract.

          Make satisfactory progress – Your grade point average does not fall below 2.0 for more than one quarter;
          must maintain a grade point average sufficient to graduate from, or receive certification in, your approved
          study; and must complete sufficient credit hours to finish the approved course of study within the time frame
          established under the approved training plan.

          Be enrolled in full time training (Full-time as defined by the training provider. Typically this is a minimum of
          10-12 credit hours per quarter for community colleges or 30 hours per week for vocational technical
          schools.) Part-time training is allowed when enrolled in RTAA, Pre-separation Training or when working in
          unsuitable employment.

          Take sufficient credits each quarter to be able to complete your training within the dates specified on your
          training contract.

          Notify your TAA Service Provider immediately if it becomes necessary to withdraw from a class. This may
          cause you to fall below full time training status and ultimately can jeopardize your TAA training funds,
          unemployment and/or TRA benefits.

          You will be responsible for arranging payment (excluding use of personal funds) for any classes you have to
          repeat due to less than a satisfactory grade (as defined by the training provider) or withdrawal.

          You are encouraged to apply for financial aid. If you are awarded financial aid, you must provide a copy of
          your award to your TAA Service Provider. Any financial aid funds awarded could be used for either your
          training except for loans (If training costs exceed the TAA limit.) or living expenses.

       I UNDERSTAND IF I DO NOT FULFILL MY RESPONSIBILITIES AS OUTLINED ABOVE, THE TRAINING PROVIDED TO ME
                             MAY BE ENDED AND I MAY BE TERMINATED FROM THE PROGRAM.
