Permit Application Package must include:
        Permit application completed and signed
Completed and signed “Disclosure Statement” or previously submitted or N/A.
Design Drawings issued, stamped and signed by a Professional Engineer registered in Arkansas.
Soils Report stamped & signed by a Professional Soil Classifier (PSC) registered in Arkansas. (Note:
PSC must be a registered Department Representative (DR) with the Arkansas Department of Health
and Human Services or a DR can work with a PSC to prepare and submit report. DR must be
        identified in soils report.)
        Location Map
        Waste Management Plan

Design Package must include:
        1. The type of system and manufacturer name (Lotus/Bioclere-Aqua Point, Orenco, etc.)

        2. Statement regarding size of septic tank(s) used for the primary treatment, if applicable.

        3. The type of collection system (pressure, gravity, or other)

        4. The volume of the septic tank(s) (gallons) and the designed retention time (days) if applicable

        5. The design flow for the DIS system in gallons per day

        6. Design flow of at least 80 gpd/capita for a STEP (Septic Tank Effluent Pumping) system, or

at least 100 gpd/capita for a STEG (Septic Tank Effluent Gravity) system, or 100 gpd/capita

            for standard gravity lines

        7. The number of lots in the subdivision

        8. The average number of people per home or dwelling used to calculate the design flow

        9. The total length (ft) of drip line in all zones

        10. The number of zones in drip field

        11. The total area (ft²) covered by drip field

        12. The size of the reserve drip field area(ft²)

        13. The calculation for the reserve drip field as percentage of operational drip field (%). The

percentage of reserve (alternate) drip field must meet the ADHHS requirements.

        14. The soil in drip field sampled and classified on 100 ft. centers
