The Nutritional Risk Questionnaire

   Core of the Nutritional Risk questionnaire are questions
    from the NSI
   Some questions modified to provide mores specific
    information about daily nutritional intake to identify
    nutritional risk.

Comparison of NSI and NR Questionnaires

NSI Question:

   I eat few fruits or vegetables or milk products.

NR Questions:

   How many servings of fruits do you eat per day?
   How many servings of vegetables do you eat per day?
   How many servings of milk, cheese, yogurt, or calcium
    rich soy products do you eat each day?

   98% were able to feed themselves
