marry or cohabit as well as their
accompanying children
This form should be used by persons applying for a residence permit and who are planning to marry or cohabit with a
person living in Sweden. This form should also be used if you are married or cohabiting with the referee but do not
have documents to certify the marriage or cohabitation. The form is also used for any accompanying children.
You should enclose this form together with the application form "Residence permit, alien's passport and residence
permit for visits longer than 90 days ('Application for RP, AP, TD and CRS')" when you apply at a Swedish mission
abroad.

K. Appendices
You must ALWAYS enclose:
− An extract from the Swedish population register for the referee, a 'family certificate' for a
residence permit which confirms the address in the population register, marital status,
children etc. The referee orders this from the National Tax Agency in Sweden and it may
be no more than one month old (if the referee is resident in Sweden).
− A copy of your passport/ID card containing your personal details.
− Extract from your country's population register containing information about marital status
and any children. (You can see what documents are required in your country on the
Swedish Embassy’s website. You can also contact the Embassy for further information.)
− A birth certificate for any children who are making a joint application.
To confirm your relationship you can also enclose:
A copy of tickets or entry or exit stamps in your passport which can confirm that you
and your partner have met.
Copies of judgments governing custody or adoption (where applicable for children
who are applying at the same time).
A copy of a marriage certificate, photographs or similar.
A copy of a pregnancy certificate.
